Santa Fe Indian Hospital, 1700 Cerrillos Road, Santa Fe, NM 87501. Hours: 8:00 am to 8:00 pm, Sunday - Saturday 2. San Felipe Health Center, 4 Cedar Street, San Felipe, NM 87001. Hours: 8:00 am to 5:00 pm, Monday - Friday 3. Santa Clara Indian Health Center, 400 State Road 30, Espanola, NM 87532. Hours: 8:00 am to 5:00 pm, Monday - Friday 4. Cochiti Indian Health Center, 270 Windmill Road, Cochiti Pueblo, NM 87072. Hours: 8:00 am to 5:00 pm, Monday - Friday Acquisition will be for a 12 month base period with (4) four 12 month option periods. |
