Cross Connection Control and Backflow Prevention Compliance Survey

This contract contains no provisions for installation, testing, or repair services. The survey shall consist of initial contact with the occupant and/or Owner of a BFP Device that includes providing an information packet supplied by the City; completion of a site survey report form to include owner/occupant signature; providing photos of the general installation and existing conditions; determination of degree of hazard as defined by WSFC Utilities based on the rules of North Carolina Department of Environmental Quality (NCDEQ); and collection of latitude and longitude coordinates. The survey of all connections shall include researching existing utility billing records to assist with determining whether or not a connection should have a BFP Device and to verify in the field that a device exists or does not exist, followed by logging the property address in a report to be submitted the city electronically.
