Northern State Parkway westbound to Interstate 495 (Long Island Expressway) westbound Ramp over Interstate 495 (Long Island Expressway), Town of North Hempstead, Nassau County 3. County Road 31 (Old Riverhead Road) over NY27 (Sunrise Highway), Town of Southampton, Suffolk County 4. County Road 3 (Pinelawn Road) over Interstate 495 (Long Island Expressway), Town of Huntington, Suffolk County 5. Belmont Avenue over NY27 (Sunrise Highway), Town of Babylon, Suffolk County 6. County Road 83 (North Ocean Avenue) over Interstate 495 (Long Island Expressway), Town of Brookhaven, Suffolk County.
