Description

Florida Department of Corrections - ADA Cell Modifications Region 1 and 2 PROJECT NAME & LOCATION: ADA Cell Modifications Region 1 and 2, Florida Department of Corrections, at the following locations: 1. Santa Rosa Correctional Institution (Milton, Florida) 2. Northwest Florida Reception Center (Chipley, Florida) 3. Columbia Correctional Institution (Lake City, Florida) 4. Reception and Medical Center (Lake Butler, Florida) This project will involve modifications to identified cells at the above listed Florida Department of Corrections Institutions to comply with the American with Disabilities Act. The scope of work will include plumbing fixture replacements, selective demolition, structural modifications, repair of concrete and masonry walls, painting, installation of steel wall panels, HVAC duct modification, limited electrical work, helical piles for foundations, and various other items. Prequalification: At least 5 days before the bid opening date prospective bidders must submit a notice of intent to submit a bid on bidder's company letterhead with a name and e-mail address for the company contact person, a copy of the company's current corporate registration with the Florida Department of State (SunBiz.org), and a current copy of State Contractor's and Business License required by DBPR for the respective trade. Bidders who are currently working on CSI Contracting, Inc. projects or have successfully completed CSI Contracting projects in the last 12 months will be pre-qualified. Please contact Scott Watson or Kendall Taylor at CSI Contracting to verify your pre-qualification, (850) 875-1471. After the bid opening, the low bidder may be required to provide additional financial and bidding qualifications in accordance with FAC Rule 60D-5.004. BID BOND: If the Base Bid or the Base Bid plus the sum of any alternates exceed $100,000, the bidder shall enclose a certified check, cashier's check, treasurer's check, bank draft or Bid Bond in the amount of not less than five percent (5%) of the Bid, payable to the Owner as a guarantee for the purpose set out in Instructions to Bidders. The bid bond must be valid until the resulting contract is executed. (Failure to submit a bid bond will result in disqualification) PERFORMANCE BOND AND LABOR AND MATERIAL PAYMENT BOND: If the construction contract award amount exceeds $100,000.00, a Performance Bond and a Labor and Material Payment Bond will be required and will be issued with the award of contract. Non-Mandatory, however bidders are strongly encouraged to attend to see the project sites, working conditions, access, learn working hours, and other special conditions involved with working inside a secure correctional setting. Because this project will involve multiple sites, there will be multiple site visits. These pre-bid meetings will be the only time to visit the sites prior to the bid opening. Failure to attend the pre-bid meetings will not alleviate the bidders' responsibility to meet the project requirements. Those who plan to attend the pre-bid site meetings will need to notify Kendall Taylor at CSI Contracting, Inc. (e-mail kt@csi-contracting.com , 850-875-1471) no later than Friday, July 8, 2022, to obtain background clearance paperwork for access to the correctional institutions. Basis of Award: The Notice of Intent to Award will be posted at CSI Contracting, Inc.'s office and bidders will be notified via e-mail. The bidder who submits the lowest price (and meets all other required bid qualifications) for the respective base bid plus additive alternates or minus deductive alternates identified will be considered the lowest responsible and responsive bidder. Please contact Scott Watson or Kendall Taylor with CSI Contracting, Inc., kt@csi-contracting.com or sw@csi-contracting.com , to obtain the required background verification form which must be submitted and approved prior to the pre-bid meeting time. Without the completed and approved background verification form attendees will not be permitted to make the site visit. DISABILITY ACCESS: Pursuant to the provisions of the Americans with Disabilities Act according to 286.26 Florida Statues, any person requiring special accommodations to participate in this meeting/bid opening is asked to advise the agency at least 48 hours before the meeting by contacting Scott Watson with CSI Contracting, Inc. at (850) 875-1471. If you are hearing or speech impaired, please contact the agency using the Florida Relay Service, 1(800)955-8771 (TDD) or 1(800)955-8770 (Voice).
