Description | During Bidding, the Architect / Owner will consider written Substitution Requests received by July 17th 2023 11:00 AM. Requests received after that time will not be considered. Submit requests in electronically on a copy of the attached Substitution Request Form via e-mail to aschaewe@k2mdesign.com. o If physical samples are required for approval then submit to Andrew Schaewe, @ 200 S. Meridian Street Suite 550, Indianapolis IN. 46225 no later than July 17th 2023 11:00 AM. o Prepare separate requests for each product. Combined requests will be rejected. Documents may be obtained only by for a fee general contract and any subcontract Bidders. Bidders will be required to provide Bid security in the form of a Bid Bond in the amount of $ 50,000.00. G. Submit your offer on the Bid Form provided. Bidders may supplement this form as appropriate. 1. Please note that bidders need to submit this completed bid form and the completed Contractor's Bid for Public Work - Form 96. H. Bids received after the stated time will be returned unopened. Any postal/courier service is the agent of the Bidder. Bids are to be submitted on the Bid Form as provided in the Project Manual and on Indiana State Board of Accounts Form No. 96 (revised 2013). Bids shall be submitted in a sealed envelope. I. A contract will be awarded to the lowest responsible and responsive bidder, complying with the conditions of the bidding documents and applicable law. The Owner, however, reserves the right to reject any and all bids, and waive any informalities, discrepancies, omissions, variances, or irregularities in bids received in its sole discretion. J. Your offer will be required to be submitted under a condition of irrevocability for a period of 60 days after submission. The project is a renovation to the existing Greensburg-Decatur County Public Library building located at 1110 East Main St. Greensburg, IN. 47240. The existing structure was built in 1993 and is a single-story steel structure and approximately 23,500 s.f. The scope of this renovation will include design services to complete interior and exterior improvements as follows: Exterior Improvements and Additions: List of Owner Selected renovations includes the following items, subject to the Owner's pre-defined budget: Site signage, clean drains, overflows, and pipes. Add riprap at site drains. Bury or lower the exposed PVC drainage pipe in the rear of the building. Repair concrete sidewalk at roof drain boot locations. Provide new concrete sidewalk/path from Community Room door to existing concrete ramp. Re-direct downspout to boot beneath new sidewalk. Replace concrete ramp at rear. Replace exterior metal railings. Provide new low-maintenance landscaping around building, trees, signs, and at new Outdoor Programming Space. Provide new Outdoor Programming space with new masonry wall with donor opportunities, trash receptacles, benches, and independent drive-up book drop in parking lot. Reconfigure concrete at front entrance for ADA compliant ramp and fully accessible front entry. Repair existing foundations and replace windows at existing bay windows, clean existing brick and limestone veneers at entry canopy, repair soffit at canopy, roofing improvements at corners. Improve security and replace exterior doors at Lobby and Community room. Interior Renovations: Renovate and provide ADA accessible areas in Men's and Women's restrooms and unisex restrooms and children restrooms as well as accessible drinking fountains. Provide updated finishes, furniture and data and A/V systems in the Community Room. Refresh interior finishes at the lobby. |
