Description | This project consists of performing preparatory work, traffic control, and installation of a: 1) Emulsified Asphalt Slurry Seal Surface Treatment; 2) ultra-thin bonded asphalt wearing surface: 3) asphalt mill and overlay for the designated portions of existing streets within the City of Osage Beach. The City anticipates issuing notice to proceed in the late summer/early fall of 2022. Bids must be submitted on the Bid Forms provided in the bound Bidding Documents Package and be accompanied by the bidder's certified check or bid bond in the amount of five percent (5%) of the total bid. A performance bond will be required of the successful bidder. The successful bidder will also be required to obtain a contractor's license and submit a certificate of insurance for workers' compensation. The prevailing hourly rate of wages, as determined by the Federal Wage Determination and/or the Industrial Commission of Missouri, Annual Wage Order #28, including amendments, will be complied with on this project, and not less than the prevailing hourly rate of wages as found by the agencies or Court of Appeals shall be paid to all workmen performing work under this contract. No bidder may withdraw his bid within 60 days after the actual date of the opening thereof. Any Bidder or equipment supplier whose firm or affiliate is listed on the General Services Administration (GSA) publication entitled Lists of Parties Excluded from Federal Procurement or Non-procurement Programs will be prohibited from the bidding process. Anyone submitting a bid who is so listed will be determined to be a non-responsive Bidder in accordance with 40 CFR 31. Bidders on this work will be required to comply with the President's Executive Order No. 11246 in regard to non-discrimination in employment. Enrollment in Federal Work Authorization program Bidders are informed that pursuant to Section 285.530, RSMo, as a condition of the award of any contract in excess of five thousand dollars ($5,000.00), the successful bidder shall, by sworn affidavit and provision of documentation, affirm its enrollment and participation in a federal work authorization program with respect to the employees working in connection to the contracted services. Successful bidders shall also sign an affidavit affirming that it does not knowingly employ any person who is an unauthorized alien in connection to the contracted services. Safety training Bidders are informed that the Project is subject to the requirements of Section 292.675, RSMo, which requires all contractors or subcontractors doing work on the Project to provide, and require its on-site employees to complete, a ten (10) hour course in construction safety and health approved by the Occupational Safety and Health Administration ("OSHA") or a similar program approved by the Missouri Department of Labor and Industrial Relations which is at least as stringent as an approved OSHA program. The training must be completed within sixty (60) days of the date work on the Project commences. On-site employees found on the worksite without documentation of the required training shall have twenty (20) days to produce such documentation. Authorized employees Contractor acknowledges that Section 285.530 RSMo, prohibits any business entity or employer from knowingly employing, hiring for employment, or continuing to employ an unauthorized alien to perform work within the state of Missouri. Contractor therefore covenants that it is not knowingly in violation of subsection 1 of Section 285.530 RSMo, and that it will not knowingly employ, hire for employment, or continue to employ any unauthorized aliens to perform work on the Project, and that its employees are lawfully eligible to work in the United States. |
