Description | The Base Bid work for the above contract includes the mill and overlay of Beth and Austin Court. The major work items and approximate quantities for which bids are requested include: 4,000 SY of asphalt milling, 1,000 Tons of HMA Pavement, 635 SF of Concrete Sidewalk (including two sidewalk accessible ramps), 900 LF of Sanitary Sewer Re-Lining, Manhole Re-Lining, spot curb replacement, driveway replacement, traffic control, and restoration. Along with the Base Bid is 12 additional bids. The additional bids are for ten (10) alleys and two (2) parking lots, with 3 full reconstructs (Robert/Highland, Chandler/Pine, Highland/Chandler - West), and the rest mill and overlays. The alleys and parking lots included are as follows: Off N Oakland Avenue (between Park Ave and Westridge Ave), Off N Oakland Avenue (between Walnut St and Summit Ave), Off N Kendrick Avenue (just north of W State Street on the west side), From S Kendrick Avenue to S Oakland Avenue (between W State St and Orchard St), From E Highland Avenue to E Chandler Blvd (between Edward St and S Perkins Blvd), From E Chandler Blvd to N Pine Street, From E Highland Avenue to E Chandler Blvd (between S Perkins Blvd and Henry St), From Robert Street to E Highland Avenue (between S Perkins Blvd and Henry St), From Robert Street to Alice Street, E Washington Street Alley (between N Pine St and N Perkins Blvd; next to Wisconsin Power Yoga), W Chestnut Street Parking Lot (across from Mill St/W Chestnut St intersection) E Washington Street Parking Lot (across from N Perkins Blvd/E Washington St intersection), Major work items and approximate quantities for all mandatory additional bids which are requested include: 2,900 tons HMA Pavement, 2,200 CY of excavation common, 3,600 TON of Base Aggregate Dense 1-1/4", and 2,000 SY of landscape restoration. These improvements include storm sewer work, clearing, replacement of concrete curb and gutter, concrete driveway replacement, sawing asphalt and concrete, excavation common, sidewalk replacement, traffic control, pavement marking, new signage, and erosion control. BIDDER'S PREQUALIFICATION: A Bidders qualification statement form will be issued with the Bid Documents. The Bidders Qualification Statement form must be completed and submitted in a sealed envelope at the time of Bid submittal. No bids will be accepted from any Bidder who has not submitted a Bidders Qualification Statement form. Contractors who have submitted prequalification statements to the City of Burlington shall state this on their Bidders Qualification Statement form. These prequalification forms will be reviewed; however, updated proof of qualification may be requested by the Owner prior to acceptance of any bid. The Owner reserves the right to reject the bid of any Bidder who has been determined to be unqualified. As a part of their Bids, Bidders shall submit a full and complete list of: 1) all of their proposed Subcontractors, and 2) the class of work which each will perform. The selected Contractor shall not alter this list without the written consent of the Owner. The PS & CD require that the selected Contractor and Subcontractor(s) furnish a Certificate of Insurance. Bidders shall submit Bids on blanks prepared for that purpose and shall state prices in writing and in figures. Use the form contained in the PS & CD. Bidders should pay attention to the requirements for Bid signatures, as set forth in Bid Security. Bidders may not withdraw their Bids within the 60-day period after the date of the Bid Opening. This requirement serves to guarantee that, if the Owner accepts the Bid, the selected Contractor will execute and deliver the proper Contract Agreement and Bonds within 15 calendar days after the Notice of Award by the Owner. Failure to return these documents within this prescribed time will result in forfeiture, to the Owner, of the selected Contractor's cash, Bid Bond, or cashier's check, as liquidated damages. The Owner reserves the right to reject any or all bids, waive or not waive any informality in the bids received and to accept any proposal, which they deem most favorable to the interests of the Owner, the City of Burlington. |
