Counties: Pinellas Road Name: SR 679 - Landscaping Limits: 15200000 Project(S): 44944215201(*) Federal Aid No: N/a Total Roadway Length: 2.670 Miles Contract Days: 860 Total Bridge Length: 0.000 Miles Total Proposal Length: 2.670 Miles Contract Execution Days: 10 Special Start Date: N/a Aquis/flexible Start Time: 90 Acquisition Days The work under this Contract consist of Landscape Enhancements along SR 679 (Pinellas Bayway) from Yacht Club Lane to Pinellas Bayway in Pinellas County. Note the substituted Commercial General Liability Insurance requirements contained in section 7-13.2 of the project Specifications. This insurance requirement is required for this proposal at time of execution. Bidders must be a "Small Business" as defined by FDOT. Small business size standards can be found at https://www.fdot.gov/equalopportunity/bdi.shtm. For more information about bidding on BDI projects, please see the training guide Prequalification per Rule 14-22 is not required. Bidders must complete and submit the Small Business Affidavit (Form No. 275-000-01) for approval prior to receipt of bid documents. It is only necessary to submit the Small Business Affidavit (SBA) form once per year. Send a completed and notarized affidavit as an attachment to: fdot.bdi@dot.state.fl.us. All Sublet work must be contracted to a small business or businesses as defined by the Department. The subcontractor must also submit a notarized Small Business Affidavit Form provided by the Department prior to the start of work. Bidders must submit the BDI Reference Sheet (Form No. 275-000-02) with the bid unless bidder is prequalified in the project's major work type, per Rule 14-22, Florida Administrative Code.
