Description | The Housing Authority of the County of Polk, Arkansas, hereinafter called the "Public Housing Agency", or PHA, will receive bids for the Exterior Door Replacement Prime bidder's returning documents prior to the bid date and/or not submitting a bona-fide bid, shall forfeit their deposit unless documents are returned seven (7) calendar days before opening of bids, with notification in writing that a bid will not be submitted. Said notification may be by email. Prime bidders who have not returned the documents by the seventh day after bid date shall forfeit their deposit. The attention of each bidder is called to the requirements as to conditions of employment to be observed and minimum wage rates (included in this Project Manual) to be paid under the contract. Participation of small, minority and women owned enterprises is encouraged. All bids must be submitted on forms furnished by the PHA as bound in the Project Manual, submitted in duplicate and submitted in a sealed envelope plainly marked to show the name and address of the bidder, bidders license no., contract job number, and date and time of bid opening. Bidders must conform to the requirements of the Arkansas State Licensing Law for General Contractors and are required to have a current Arkansas Contractor's License in accordance with current Arkansas State Statute 71-701 et seq. Each bid submitted must contain the following items. 1.Bid Form included in the Project Manual, with all blanks filled in, signed by bidder 2.A cashiers check, certified check, or acceptable bidders bond payable to the Owner in an amount not less than 5% of the largest possible total for the bid submitted must accompany each bid as a guarantee that, if awarded the contract, the bidder will promptly enter into a contract and execute such bonds and insurance as may be required. 3.A fully executed Non-Collusive Affidavit as included in the Project Manual 4. A fully executed Previous Participation Certification, HUD-2530 included in the Project Manual 5. A fully executed Representations, Certifications, and Other Statements of Bidders, HUD-5369-A included in the Project Manual, including: A. Bidders Section 3 Clause Certification B. Certification of Bidder regarding Section 3 and Segregated Facilities C. Certification of Prime Bidder regarding Equal Employment Opportunity D. Certification of Prime Bidder regarding Non-segregated Facilities E. Contractors Certification regarding Labor Standards and Prevailing Wage Rate Requirements. The successful bidder shall be required to furnish an Arkansas Statutory Performance and Payment Bond or Bonds in the amount of 100 percent of the Contract Bid for contracts that exceed $25,000. No Bidder may withdraw his bid within sixty (60) days after the actual date of the bid opening. The PHA reserves the right to reject any or all bids or to waive any informalities in the bidding. |
