Description

Maintenance and cleaning of two (2) fish aquarium tanks at the Department of Veterans Affairs John D. Dingell Medical Center located at 4646 John R. Street, Detroit, Michigan 48201. Tanks are located in Ward A5 South and Ward A6 South, in the Blue Section of the VA Medical Center on the 5th & 6th Floors. Service shall be provided for a base year and shall include options for four (4) additional years of service. Options shall be exercised at the Government s discretion. Maintenance shall include: Site visits once per month, same day and time for the life of the contract. Hours of Work: Work shall be performed Monday through Friday between the hours of 7:00 A.M. and 4:30 P.M. Cleaning of two (2)-75-gallon fish aquarium tanks to include: Cleaning inside & outside of glass. Cleaning of gravel. Cleaning of filter system (Penguin BIO-WHEEL 330). Provide any water tests/treatments as necessary. Adjusting plants, rocks, and general underwater scenery. Replacement of aquarium lights as needed. (burnt out, broken, malfunctioning, Sylvania Cool White , 15 Watt, F15T8/CW, A468,0 46135 21616 9) Replacement of the filtering materials. Contractor shall provide fish food appropriate for fish species. Tank A5S currently has approximately 12 exotic fish and tank A6S has approximately 8 exotic fish. Contractors Personnel Security Requirements: The Information Security Officer (ISO) requires that all contractors employees working on the contract shall be required to take the VA privacy training prior to starting work. Government s Responsibilities: The Government will provide access to any rooms or equipment where contractor needs to perform work. Contractor shall sign the Sign-In Sheets located at both aquariums and provide a field service report documenting work performed on each visit. Failure to sign in and leave a field service report may result in non-payment or delay in payment. Contractor shall collaborate with the Contracting Officer s Representative (COR) regarding overall services provided.
