Line # Scope of Work Item Code Estimate EXTERIOR HOUSE 1 Soffit Repairs: Repair or replace all damaged and loose soffit boards. Apply paintable caulking at all gaps and cracks. At the repaired areas, spot prime paint and finish paint, The work presented in this project is to be started within 10 days after Contractor's receipt of a Notice to Proceed and is to be satisfactorily completed within 60 days thereafter. Interior/Exterior Completion and/or Weather-Affected repairs, if any, must be completed as further outlined and incorporated in Bid Attachments A and/or B respectively. (Strike if not applicable.) to match existing. CV, Pb 2 Fascia Repair and Wrap: Repair or replace all damaged fascia and boards. Permanently repair and cap-over any rodent holes with metal flashing, wherever found. Wrap the fascia in pre-finished aluminum wrap. Colors per owner. CV, Pb 3 Gutters and Downspouts: Replace all gutters and downspouts with 5" gutters and 4" downspouts. Re-slope the gutter system, and relocate the downspout locations, so that the roof discharges into the front yard, and not into the rear or side yards. Direct downspout discharge to grade with bends and extensions, a minimum of 4' away from the foundation. Provide splash blocks. Install permanent caps at abandoned storm inlets. Colors per owner. CV, Pb 4 Chimney Repairs: A.) Parge the brickwork of the chimney; Install galvanized metal mesh, and two coats (minimum) of cementitious coatings. B.) Re-flash the chimney with metal flashing color matched to the roof. C.) Install a metal rain cap with a bird/bug screen. C V 5 Window Glass Repair: Replace the broken glass on one West facing window. CV, Pb INTERIOR GENERAL 6 Alarms: Install a combination smoke/CO alarm in the basement, and first floor. All alarms shall be sealed battery, 10 year lithium ion units. C V Miscellaneous: The contractor is responsible to field verify all measurements, the amount of materials needed, and the number of windows in the building. If any item in this scope calls for a certain manufacturer, model number, or approved equivalent of a particular item, and that item is to be substituted, both NIDC and the owner must approve the substitution in writing as a part of the contract. Both the "Rehabilitation and Technical Specifications and Performance Standard for the City of Milwaukee - February, 2006" and the "NIDC Addendum to Specifications, " dated 8/23/07 & 5/13/08, are incorporated into this scope by reference. These items provide an outline of materials requirements & performance expectations. Updated copies can be obtained from NIDC @ 809 N Broadway-3rd Flr. Lead Related Work: Any task that a disturbs previously painted surface shall be performed by properly certified personnel and follow the State of Wisconsin Department of Health Services Administrative Code Chapter DHS 163 regarding the certification for the identification, removal and reduction of lead-based paint hazards (Pb). And, if applicable: Any task that disturbs a previously painted surface requires Milwaukee Health Department notification (Pb-N). Window replacement shall be performed by properly certified personnel and requires a Milwaukee Health Department Permit (Pb-A). YES NO BASEMENT 7 Dryer Vent: Replace the flexible dryer vent with a solid walled galvanized vent pipe. C V 8 Electrical Repairs: Properly terminate electrical wires on open junction/fixture boxes (wire nuts, etc.) and Install outlet, switch, and junction box covers, at all locations where missing. Install an NM clamp at one location where an NM wire enters a ceiling junction box through an open knock-out. C V 9 Sewer Lateral: Repair the cracked sewer lateral. Work with the owner to view all images of the lateral, or conduct another camera scope, if necessary. (PERMIT), C V 10 Stair Handrail: Install a code compliant handrail on the stairs. Finish paint. C V 11 Sump Crock Cover: Install a new sup crock cover, and dispose of the existing (cracked) concrete cover. C V FIRST FLOOR 12 Shower Surround: Remove the existing tile and wall surface down to the studs. Install cement board panels, and coat with Redgard waterproofing, then install owner approved tile. All perimeter edges of the tile surface are to be terminated with a bullnose edge. All joints and seams are to be caulked with color-matched silicone sealant. CV, Pb BASE BID SUMMARY (Note: Contractors are to provide line item estimates with their bid.) Scope Estimate: ALTERNATE WORK ITEMS 13 South Addition, East Wall Base-Flashing Repairs: Replace the bent and loose base flashing, and secure it to the foundation. Repair the siding along this flashing; Replace rotten boards, and repaint (to match existing) the exposed wood areas, where paint has peeled away. CV, Pb 14 South Addition, West Wall Repairs: There is an unprotected cavity at grade, concealed behind the siding, along the West side of the addition. Rodents are using this area to gain access into the structure of the house. Cut the wall's board-andbatten siding up from the finish grade, to expose the concealed cavity. Patch any discontinuous thermal insulation. Install treated wood boards and galvanized metal flashing to seal off the structure, and make it rodent proof. Repair the siding; Install matching trim work and paint to match existing, for a finished complete appearance. CV, Pb 15 Roof Replacement: Tear off all layers of existing shingles and replace the entire roof. Replace any underlying rotten or damaged roof sheathing, and install 7/16" OSB sheathing over the entire deck. Install 'W' flashing at all valleys. Provide ice and water shield a minimum of 3 feet wide at all roof edges, and valleys, install synthetic roofing felts, and metal drip and apron flashing. Install new roof vents per code. Provide all necessary flashings, and minimum 30 year dimensional shingles.
