CV, PbN EXTERIOR (HOUSE) PORCHES 9 Second Floor Rear Porch - Make all necessary repairs to the guardrails and support columns to meet city codes. CV, (PERMIT) 10 Second Floor Rear Porch - Replace defective EPDM rubber and flash to the house at 6" minimum. Lead Related Work: Any task that a disturbs previously painted surface shall be performed by properly certified personnel and follow the State of Wisconsin Department of Health Services Administrative Code Chapter DHS 163 regarding the certification for the identification, removal and reduction of lead-based paint hazards (Pb). Any task that disturbs a previously painted surface requires Milwaukee Health Department notification (Pb-N). Window replacement shall be performed by properly certified personnel and requires a Milwaukee Health Department Permit (Pb-A). Miscellaneous: The contractor is responsible to field verify all measurements, the amount of materials needed, and the number of windows in the building. If any item in this scope calls for a certain manufacturer, model number, or approved equivalent of a particular item, and that item is to be substituted, both NIDC and the owner must approve the substitution in writing as a part of the contract. CV 25 Gutters and downspout s - Repair and realign defective gutter system and downspouts. CV, PbN INTERIOR - GENERAL 26 Alarms - Install Carbon Monoxide and Smoke Alarms as required by the city code. Alarms shall have 10 year sealed lithium battery. All sleeping rooms shall have a 10 year sealed battery smoke alarm. CV 27 Doors - Replace defective or missing door and door components as necessary (knobs, hinges, locks, jambs, etc.). This line item covers the entire structure. CV 28 Electrical - Make all necessary repairs to make the dwelling code complaint and safe to be occupied. Upgrade receptacles in the kitchens, bathrooms and basement to GFCI. Check electrical panel and properly ground the system. All lights, receptacles, breakers are required to be operational. Missing cover plates for outlets and switches are required to replaced. Improper wiring shall be corrected to meet electrical code. PR, (PERMIT) 29 Plumbing - Natural Gas Lines - Make all necessary repairs to make the dwelling safe to be occupied. Unused lines shall be capped. CV, (PERMIT) 30 Plumbing - Make all necessary repairs to make the dwelling plumbing to meet code requirements and safe to be occupied. Replace defective or missing pipes in the basement, first and second floor unit. Black and gray water pipes are also included in this line item. Replace missing or defective grills in the air return and supply. PR 34 Molding - Replace missing wall base molding and trim molding as necessary. PR, PbN 35 Surface Paint - Prepare, prime and paint walls and ceilings. Color to be determine by property owner. This line item includes closets, bedroom, hallways, bathroom, etc. PR, PbN 36 Ceilings - Replace missing and defective ceiling tiles. CV 37 Insulate - insulate second floor walls prior to installing new drywall. CV 38 Floors - Detail clean all floor surfaces through out the structure. Replace missing or defective floor material. This line items includes hallways, bathrooms, kitchens, bedrooms, etc. Also replace buckled flooring material on the second floor. CV BATHROOM UPPER 39 Pedestal - Correct pedestal installation. CV 40 Seal - apply silicone caulking around bathtub. PR 41 Flooring - Replace flooring tiles. PR, (PERMIT) BATHROOM LOWER 42 Walls - Replace missing and defective plaster on the walls. Prepare, prime and paint. PR, PbN 43 Ceiling - Replace missing and water damaged ceiling tiles. CV 44 Medicine cabinet - Replace defective medicine cabinet. PR 45 Floor - repair or replace flooring. PR KITCHEN - UPPER 46 Cabinets - Make all the necessary repairs to make the cabinets functional and clean. PR 47 Cabinet upper - under the sink. Repair plaster and cover opening. CV 48 Floor - Clean tile floor. PR 49 Walls and ceiling - Prepare surfaces, prime and paint. PR, PbN KITCHEN - LOWER 50 Cabinets - Make all the necessary repairs to make the cabinets functional and clean. PR 51 Sink - Supply and install plug. (water sprayer outlet). PR 52 Under sink cabinet - Replace P-trap, tail pipe, cover opening, repair plaster, prime and paint.
