Description | The City of Middletown is requesting written proposals for providing all professional services listed herein for the design of the Flemming Road Water Main Replacement project. The Flemming Road water main replacement project will include full depth restoration for the roadway. The intent of this task is to provide an accurate representation of existing topography. The survey shall contain the necessary information to design the proposed improvements to Flemming Road but not be limited to, the following: 1. Topographical survey shall include points within a 100 foot corridor, 50 feet on each side of existing centerline of Flemming Rd from University Blvd. to the dead end, east of Sierra Rd including all intermediate intersections. Survey at intersections shall extend 75 feet past Flemming Road edge of pavement. Utility locates will be required. 2. Document existing property and right-of-way monumentation along the roads. Resolving the right-of-way and property boundary are not part of this project. 3. Semi-permanent control points with X, Y and Z coordinates no further than 500 feet apart. 4. All points/shots with point numbers, descriptions and elevations, including storm sewer and sanitary sewer data. 5. Digital Terrain Model (TIN) with contours, and a separate 2D file of topography (Z=0) on ODOT template base file. 6. Temporary BMs along the project and outside the work limits. No temporary bench marks set on existing utilities and are to be placed no further than 500 feet apart. Control points and bench marks can be one and the same. 7. Line work of all existing utilities, driveways, walks, culverts, buildings, fences, lots, valves, trees (including diameters), utility poles, and all underground utilities, etc. shall be per ODOTs CADD requirements. 8. Copies of all documents. 9. The final product or drawings shall be submitted in an AutoCAD 2016 or later format reflecting ODOT line styles, layers, blocks, patterns, symbols, plot style table, etc. and provide any technical assistance needed to upload data into AutoCAD. 10. At return radii where ADA ramps exist, detailed elevation shots should be obtained at existing ADA and walk joints up to a distance of 10+ feet from the edge of the ramp. The City will provide the following: 1. Benchmark information. 2. Any available as-builts in hardcopy or digital format. 3. Aerial photograph to use for a background base. 4. ODOT AutoCAD template base file. 5. If any catch basin grates or manhole lids are resistant to opening, city crew can assist in opening with advanced notice. The duties and responsibilities of Consultant during the Projects Design are identified as follows: Prepare 50% design deliverable, including drawings and table of contents for the project manual. Drawings shall provide the proposed plan and profile view location of approximately 5,905 feet of new 8-in water main, valves, hydrants, services, and other associated appurtenances necessary for replacement of the existing 6-inch pipe. Consultant shall meet with City to review and discuss 50% design deliverable Prepare 90% design deliverable, including drawings, sub-summaries and general summaries with item numbers and labels, bid form and necessary technical specifications, and draft temporary and permanent easements as necessary. Drawings shall provide the proposed plan and profile of approximately 5,905 feet of new 8-in water main, valves, hydrants, services, and other associated appurtenances necessary for replacement of the existing 6-inch pipe. Consultant shall meet with City to review and discuss 90% design deliverable. Prepare bidding documents including 100% complete drawings, bid form and necessary technical specifications, and final temporary and permanent easements. Consultant shall assist the City as follows: Attend pre-bid and bid opening meetings Answer written questions received from Contractors 3 Prepare up to two addenda Evaluate bids and prepare bid award opinion Consultant shall attend one pre-bid meeting to discuss the project with prospective bidders. Consultant will also attend the bid opening and provide a bid evaluation letter that summarizes the line item bid costs and recommendation for award of a construction contract. To support the performance of the Scope of Services described above, the City will conduct the following activities: Provide existing water line drawings in digital format. Contact property owners and obtain permission regarding access for field visits and equipment access. Provide property acquisition negotiations for easements, purchase, or lease. Review data and reports prepared by Consultant and provide review comments in a timely manner. Pay for any regulatory agency plan review and approval fees. Incorporate Consultants Final Deliverable into the Citys standard Contract Document format for issuing during bidding. Advertise to and solicit bids from prospective bidders. Reproduce, manage, and oversee the distribution/selling of Contract Documents to prospective bidders. City of Middletown sheet borders. |
