Description | Statement of Work (Includes Specifications) This SOW describes the Third Party Commissioning (New Building 5) at Richard L Roudebush VA Medical Center, 1481 West 10th Street, Indianapolis, IN 46202. This Statement of Work describes the requirements for the inspection, testing, and commissioning of the new Building 5 electrical equipment and electrical connections, elevator system, AHU/HVAC equipment and duct installation, domestic water lines and associated equipment, sanitary sewer lines, and stormwater sewer lines located at the Richard L. Roudebush VAMC, 1481 West 10TH Street, Indianapolis, IN 46202. Specific Requirements. The contractor shall provide all tools, equipment, material, labor, and supervision necessary to accomplish the following: Review all 583-331, Expand Specialty Care Clinic Project drawings specifications, and relative submittals as they relate to the new Building 5 to ensure that the contractor has installed all new equipment and utilities as mentioned below per the contract bid documents and to meet compliance with NFPA, NEC, and all Indiana building codes. Inspect/test all electrical equipment and review all electrical connections (HV/MV/LV) to ensure that all installation work meets compliance with the latest NFPA/NEC/submittal requirements. Inspect/test the elevator system to ensure that all installation work meets compliance with the latest ANSI/NFPA/NEC/submittal requirements. Inspect/test the AHU and all associated equipment, check for duct leakage in all ducts, and review the AHU TAB report to ensure that all rooms within the new Building 5 have been properly tested and air balanced. Review all mechanical/electrical equipment to ensure that all equipment has been properly installed and meets compliance with contract specs/drawings/submittals. Inspect/test the domestic water lines within the new Building 5 to ensure that they have been properly installed, flushed of all debris, and meet compliance with contract specs/drawings/submittals. Check all strainers and filters for debris. Inspect/test the sanitary sewer lines that are connected to the new Building 5 to ensure that they have been properly installed, flushed of all debris, and meet compliance with contract specs/drawings/submittals. Inspect/test the stormwater sewer lines that are connected to the new Building 5 to ensure that they have been properly installed, flushed of all debris, and meet compliance with contract specs/drawings/submittals. Provide a detailed report listing all items/systems that have een reviewed/tested with associated photos and provide recommendations for correction for all work described above. Provide specific specification/drawing note/code references for any work that does not meet a specific specification/drawing note/code requirement. Review and complete all information required by the associated attachments to this scope of work. Locations. 3.1 1481 West 10th Street Campus. The scope of this project shall include all work as described above at the Richard L Roudebush VAMC, 1481 West 10th Street, Indianapolis, IN 46202. Qualifications of Contractor: Contractor shall be certified to inspect, test, and commission new buildings and building utility systems. Contractor shall have completed the Occupational Safety & Health Administration (OSHA) approved 10-hour construction safety training. Project superintendent shall have completed an OSHA approved 30-hour construction safety training. Contractor shall provide evidence of having technical training and documented work experience in inspecting, testing, and commissioning of new buildings and building utility systems. A minimum of three years of experience is required. Contractor shall be equipped with all necessary tools, equipment, materials, labor, supervision, and Personal Protective Equipment (PPE) necessary to perform all required work in a safe, effective, and timely manner. Tools, equipment, and PPE shall comply with the requirements of OSHA Standard 29 CFR 1910, Subpart I, and NFPA 70E. Work Hours Maintaining the operational capability of the medical center is of paramount importance. The contractor shall coordinate all activities with the VA COR prior to commencing any work. To maintain the medical center s operational capability, this project will require working off-shift hours as well as weekend hours when necessary to shut down existing utilities. Work can be accomplished during normal working hours as long as it is not disruptive to the staff within the new Bldg 5. Normal business hours for the medical center are defined as 0800 1730, excluding Saturday and Sunday. Designated off-shift hours are defined as 1730 0600, Monday through Friday, excluding federal holidays. Infection Control Risk Assessment (ICRA) Requirements Contractor shall comply with all PCRA/ICRA/ILSM requirements for work associated with inspecting, testing, and commissioning of the new Building 5 and the new Building 5 utility systems. Summary of Work 7.1 Inspection, testing, and commissioning requirements shall be based upon manufacturers recommendations for each component and/or the industry standard. 7.2 VA will provide the 583-331, Expand Specialty Care Clinic Project drawings, specifications, and submittals for the new Building 5 and utility systems to the contractor electronically. Period of Performance Contractor shall complete all specified contract work and provide the final commissioning report/red-lined drawings to the VA COR within a 15- calendar day timeframe from issuance of the Notice to Proceed (NTP) from the Contracting Officer. Deliverables: Contractor shall provide a detailed commissioning report with photos showing the new Building 5 equipment and utility systems that were inspected and tested along with all recommendations for correction. Provide the commissioning report in hardcopy form (3 copies) and provide red-lined drawings in hardcopy form (3 copies). In addition, provide the commissioning report in Word/PDF format on a CD. Submittal Instructions: All interested and capable vendors must submit an electronic copy of their SF330s, via email, by the closing time/date of this announcement. No hard/physical copies will be accepted. They must be submitted to the contracting officer at luke.turner@va.gov. Evaluation Criteria: The government will evaluate each potential contractor under the terms of its: Professional qualifications necessary for satisfactory performance of required services; (2) Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction, and the use of recovered materials; (3) Capacity to accomplish the work in the required time; (4) Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules; (5) Location in the general geographical area of the project and knowledge of the locality of the project; provided, that application of this criterion leaves an appropriate number of qualified firms, given the nature and size of the project; and (6) Experience in construction period services to include professional field inspections during the construction period, review of construction submittals, support in answering requests for information during the construction period, and support of construction contract changes to include drafting statements of work, and cost estimates. A selection board has been appointed to complete the functions required by FAR 36.602-3, under the selection process outlined at FAR 36.602. ***END OF ANNOUNCEMENT*** The method of contractor has not determined at this time. |
