Description

Fairfax CDBG REAP Water System Improvements project, to wit: BASE BID 1 1 LS MOBILIZATION 2 1 LS SN SERIES IN-TANK SPRAY AERATION SYSTEM, SN10 3 1 LS HEADSPACE VENTILATION BLOWER, F4: 4 1100 LF 8" SDR11 HDPE WATERLINE 5 1 LS CONNECT TO 6" WATERLINE 6 1 LS DISINFECTION AND CHLORINATION NOTE: CONTRACTOR TO INSTALL TEMPORARY BLOW VALVE TO FLUSH LINE IF NECESSARY. 7 1 EA 8" GATE VALVE & BOX 8 2 EA RECONNECT TO EXISTING METER 9 2 EA 6"x8" DUCTILE IRON TO HDPE TRANSITION WITH WALL ANCHOR 10 1100 LF PIPE BURST 6" DIP 11 1 EA 6" GATE VALVE AND BOX 12 1 LS CONSTRUCTION STAKING Small business and minority owned business sources are encouraged to bid on this project. A bid bond in the amount of 5% of the total amount bid shall be attached to all bids as a guarantee that the successful bidder will within ten (10) days from the date of notice of award, enter into contract and file the required bonds, and furnish a certificate of Workman's Compensation and General Contractors Liability Insurance. The Town of Fairfax reserves the right to reject any and all bids, waive any irregularities or technicalities without assigning any reason therefore, and to make the awards in the best interest of the Owner. The Town of Fairfax reserves the right to withhold any contract awarded subject to the availability of funds. Non-Collusion form must be signed, notarized and returned with bid or bid will not be accepted. All bids must include assurances that the following provisions will be complied with: Federal Labor Standards Provisions U. S. Department of Labor 29 CFR Part 5; Section 3 of the Housing and Urban Development Act of 1968, as amended, 12 U.S.C. 170IU which requires that to the greatest extent feasible opportunities for training and employment be given lower income residents of the Town of Fairfax and contracts for work in connection with the project be awarded to business concerns which are located or owned in substantial part by persons residing in the area of the project. Section 109 of the Housing and Community Development Act of 1974 which assures that no person shall on the grounds of race, color, national origin or sex be excluded from participation in, be denied the benefits of, or be subjected to discrimination; Certification of Non-segregated Facilities which assures that bidder does not maintain or provide any segregated facilities; Equal Opportunity Provision, EO 11246, as amended, which assures non discrimination; Minority Business Enterprise and Women Business Enterprise Provisions which encourage minority owned businesses and women owned businesses to paid on the project; assurances that surety companies executing bonds appear on the Treasury Department's list and are authorized to transact business in the State of Oklahoma. Contracts for this construction will require the submissions of a preliminary statement of anticipated work force and subcontractor needs for project construction work, as well as the submission of written goals (number of employees and dollar amount to subcontractors) for utilization of lower-income residents and small and disadvantaged businesses located within Osage County. The Town of Fairfax reserves the right to reject any of all bids, waive any irregularities or technicalities without assigning any reason therefore, and to make the awards in the best interest of the Owner. (Bid shall be awarded to the lowest, responsive and responsible bidder). No Bidder may withdraw his bid within 60 days after the actual date of the opening thereof. Bids are solicited and a contract award will be made pursuant to the Public Competitive Bidding Act of 1974 as amended (Title 61, O.S. 1981, Sections 101-136 et seq.). **A contractor must be registered and updated in the SAM. gov system as this review is conducted by the Owner before any contract award is executed. ** The contractor must obtain plans and specifications from the office of the Engineer and be registered on the Engineer's bid list to be recognized as an eligible bidder for this project.
