Description | Repaving of the Fire Station parking lot. This will involve removal and disposal of old material and repaving and striping; approximate area is up to 11,500 sq/ft. The second project (2) is installation of approximately 200 ft of electrical conduit with five junction boxes and LED lights, tied into existing electrical system. (1) Project involves: a. Remove and replace (once project is complete) one Conex box; b. saw-cut parking lot edges as required, remove existing asphalt to 27075 S Canoa Road, Amado AZ 85644; c. grade and compact subgrade; d. grade parking lot to ensure proper drainage; e. asphalt Paving (no more than 11,526 SF) 2" at parking lot and 3" at entries; f. tack coat vertical edges, furnish, deliver and install an average compacted thickness of two inches of PAG specification hot mix asphalt #2; g. install approx. 50 LF of 6" x one foot headers at single entry; and, h. restripe parking lot (2) Project involves: a. Installation of approximately 200 linear feet of electrical conduit of suitable type, south side of existing building b. Installation of five junction boxes and LED light assemblies c. Tie all to existing electrical system In addition, related project services include but are not limited to the following: Provide contractor management services during construction including but not limited to obtaining all required permits and permit drawings, scheduling inspections, coordinating scheduling, and providing construction cost estimates, phasing plans and related schedules where applicable; oversee and maintain quality control; maintain proper safety practices in accordance with OSHA standards; communicate work schedule with designated Elephant Head Volunteer Fire Department staff; comply with the Davis-Bacon Act including providing required paperwork and acceptable certified payrolls; and, ensure demo and disposal of materials/debris in manner noted at (1)b. A mandatory pre-construction meeting with the awarded contractor/subcontractors, Elephant Head Volunteer Fire Department, and representatives from Pima County`s Department of Community & Workforce Development will be required prior to the issuance of any Notice to Proceed for these projects.�This meeting will be scheduled in consultation with the selected contractor(s).� The selected firm/s will be expected to work collaboratively with Elephant Head Volunteer Fire Department and shall have previous experience in the applicable trade/s as it relates to the scope of work, follow applicable local, state, and federal codes, and be licensed, bonded, and insured. All work shall be performed in a safe and professional manner and all waste material removed and work site cleaned as applicable. SUBMITTAL INFORMATION To be considered for this project, the following items shall be submitted: 1. Statement of Qualifications, portfolio, or similar including the following: Firm name, address, phone/email, brief history and services offered by your firm; services to be provided by your firm and services to be provided by outside subcontractors (including name and license number) on this particular job; related experience describing projects of a similar scope and nature with date of completion; State of Arizona licenses as applicable; at least three specific references that may be contacted including name, title, company affiliation, mailing address, email address; and, identification of a lead management person who will have day-to-day responsibility for this project. This person should have the demonstrated experience and time available to manage the entire project. |
