Description | Furnishing the material and performing the labor for the execution of: ERISR4-10.66 Hayes Ave in accordance with the Plans and Specifications prepared by Director of Public Works, on file in its office, 240 Columbus Ave., Sandusky, OH 44870. All questions shall be directed to Jane Cullen, P.E., Assistant City Engineer in writing via email at jcullen@cityofsandusky.com no later than Monday, November 14, 2022 at 8:00am. The successful bidder MUST be an Ohio Department of Transportation (ODOT) prequalified contractor at the time of bidding Prevailing Wage Rates under the Davis-Bacon Act, Federal Labor Standard Provision and the Equal Opportunity Requirements are applicable to the Contract. There is a 6% DBE requirement that must be met on this project. Please refer to Section 25 titled: PN 013-03/15/2019-DISADVANTAGED BUSINESS ENTERPRISE (DBE) UTILIZATION PLAN AND GOOD FAITH EFFORTS. If the contractors are not able to submit via Formstack, they must submit a DBE Utilization Plan to this email: dot.contractslettingmgr@dot.ohio.gov prior to bid opening. They must include their firms information, PID number for the project, total bid amount they will be submitting, and list each DBE they are planning on using along with the dollar amount for that DBE. Also, to ensure goal will be met they will need to indicate if they are using them as a subcontractor, supplier (60%), etc. Again, these emails will need to be received prior to bid opening with all the completed information. The required contract provisions for federal-aid construction contracts contained in ODOT's 2019 LPA template are hereby incorporated by reference. The rules and regulations shall apply to all work to be done under this contract. If any provisions of these rules and regulations conflict with any other clauses of this contract, the ODOT 2019 Construction and Material Specifications shall govern. |
