Conductor Hole for Forge

1) Scope of work a) This procurement is for a fixed-price (turnkey) operation to excavate a cellar, drill a hole (hereafter referred to as “16A(78)-32 conductor”) and to complete the hole as outlined in section 4. b) 16A(78)-32 conductor is to be vertical, 100-foot deep and a minimum of 30 inches in diameter. c) Contractor shall obtain all permits required for mobilization and demobilization. d) FORGE has been informed by the Utah DWR that no drilling permits are required for this work. e) Contractor shall include in its bid package a copy of its safety program, records of any safety incidents in the past two years (e.g., OSHA 300 reports), and its safety inspection checklist. f) Compliance with API and Utah Division of Occupational Safety and Health standards is required. FORGE may request UOSH field inspection. g) Any fishing operation is the responsibility and at the expense of Contractor. h) This document does not cover HSE issues – All HSE and spill procedures will be consistent with DOE approved HSE procedures and requirements.
