Description | Provide labor, materials, equipment, and services necessary for floor refinishing services, on an as-needed basis, for the Housing Authority of the City of Milwaukee (HACM) - Market Rate Developments (2-story apartment homes). Berryland Office 414-286-8851 located at 6089 N. 42nd Street, Milwaukee 53209 Northlawn Office 414-286-8864 located at 5145 N. 20th Street, Milwaukee 53209 Southlawn Office 414-286-8867 located at 3350 S. 25th Street, Milwaukee 53215 The Contractor shall have in its employ a sufficient force of qualified and competent personnel to commence work on each dwelling unit within five days after notification by the HACM to proceed. Picking up unit key(s) from the development office does not constitute commencement of work. The Contractor shall complete the work within seven working days. The housing manager may grant the Contractor up to an additional seventy-two (72) hours to complete the work. If work is not completed within seventy-two (72) hours (or within the additional time extension granted by the housing manager), liquidated damages in the amount of Fifty and 00/100 Dollars ($50.00) may be assessed for each day of delay. The Contractor shall receive authorization to proceed with work via a Work Order. Work Orders will be issued by the Manager. After completion of the work order the contractor shall return the Work Order to the Manager. When keys are not submitted with the Completed Work Order to the development office, the Contractor may be assessed a key and lock change fee of One Hundred and 00/100 Dollars ($100.00) per two-door unit. These fees are non-refundable. Contractor shall act as General Contractor and shall include all labor, material, equipment and work of all other trades in his/her bid as necessary to complete work identified as: Sanding and Sealing Hardwood Floors and as indicated in these specifications. Contractor shall coordinate all trades and insure that all work is completed within the time frame set forth stated on the Request for Services Pricing of the Bid. Work shall be scheduled so that at time of commencement of work all labor, materials and equipment are available at the site. Work must be pre-scheduled with the Housing Authority and commence only as prearranged. The Development Manager shall contact the Contractor of impending work via a work order. The Contractor shall return the work order to the development office within 48 hours with a tentative commencement date no later than five days. Work to be completed within seven working days. Contractor to contact development office within 24 hours prior to arriving at the site. If Contractor arrives at development, after 24-hour prior notification, to commence with work and the existing conditions make it impossible to do the work, the Contractor is to leave a copy of the work order with existing deviation of the work or fax/email the development office with it. The Development Manager to contact the Contractor when existing deviation has been corrected and work can commence. Contractor to reschedule within the next five working days. Contractor shall notify Development Manager when work is completed and arrange for final review of work. |
