Description | The Hampton VAMC has a requirement for a contractor to provide all transportation, materials, parts, tools, specialized equipment, labor, supervision and technical expertise to construct a concrete driveway and gas line protection for the roll up door entrance to Building 15. FAR 36.204 Disclosure of the magnitude of construction project: (b) Between $25, 000 and $100,000.00 NAICS: 237310 Highway, Street, and Bridge Construction Size Standard: $39.5 Million Annual Revenue 100% VOSB Set aside All submitted quotes must be received by e-mail by the closing date and time. Quotes shall be submitted via e-mail to toni.knight@va.gov. No other method will be accepted. A Site Visit is scheduled for: September 12, 2022 at 10:00 AM outside of Bldg. 27 RVSP for the site visit NLT 1:00 PM on September 8, 2021 Inquires and/or Questions for project must be submitted via email to Toni.Knight@va.gov NLT September 16 by 3:00 PM EDT RFQ Amendment with inquire responses will be posted to sam.gov on September 19 2022. Submission of quotes are due on or before October 5 2022 at 10:00 AM EDT. STATEMENT OF WORK Install Concrete Driveway and Gas Line Protection for Building 15 Roll-up Door Entrance Project 590-22-2-583-0137 VA Medical Center Hampton Virginia 1.0 GENERAL REQUIREMENTS The Hampton VA Medical Center (VAMC) located at 100 Emancipation Drive, in Hampton, Virginia requires a contractor to provide all transportation, materials, parts, tools, specialized equipment, labor, supervision, and technical expertise to provide a concrete driveway and gas line protection for the roll up door entrance to Building 15 that accesses Emergency Contingency Supply Storage Space for the Engineering Service, Safety Office, Emergency Manager. The following requirements apply to this work: 1.1. This service will be a firm-fixed price contract. 1.2. Upon award, the Contractor Shall provide all transportation, materials, parts, tools, specialized equipment, labor, supervision, coordination, and technical expertise to perform the work outlined in this SOW to deliver a fully functional concrete driveway suitable for supporting the operating weight of a loaded forklift that allows the forklift to safely access the rollup door to the elevated storage space of Building 15. Additionally, the contractor shall provide the same services outlined above to provide a protective steel cage around the natural gas line that runs the width of the rollup door such that the forklift is unable to contact, penetrate, or otherwise damage the natural gas line when being utilized to move supplies into the boiler plant. 1.3. The Contractor will provide for contracted utility marking of the construction area. 1.4. The Contractor shall mobilize equipment and materials to the site. 1.5. The Contractor shall excavate approximately 550 SF area to minus 8" and haul spoils offsite. 1.6. The Contractor shall review with the VA COR the best way to provide positive drainage from the proposed concrete slab to determine the best possible way to provide drainage from the site while minimizing cost and environmental impact. The final slab drainage plan and method shall be approved by the VA COR. 1.7. The Contractor shall install HDPE pipe for drainage under slab. 1.8. The Contractor shall grade, form, and pour a 6" concrete slab. 1.9. The Contractor shall remove forms and grade site and slab to match the existing/surrounding area. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document. |
