Description

As of August 18, 2022, project is on hold. No definite date for the rebid. Bathroom Renovation (1FL) This is an RFQ (request for quotes). This is not an IFB (bid). The Metro DDSOO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate relevant experience on prior projects of similar magnitude. The Metro DDSOO reserves the right to reject any and all quotes. In deciding whether to or not to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker's Compensation Insurance as well as completed Lobbying Law Forms, and Signed Appendix A. Please be prepared to submit these in a timely fashion. The Metro DDSOO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition. The Metro DDSOO's interpretation of specifications shall be final and binding. Please note all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitors log and/or an attendance sheet provided by the maintenance department. NYS Labor law requires that all contracted employees performing maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or a building service contract covered by Article 9 thereof, neither Contractor's employees nor the employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law and as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Furthermore, contractor must pay at least the prevailing wage rate and pay or provide the prevailing supplements, including the premium rates for overtime pay, as determined by the State Labor Department in accordance with the Labor Law. NYS law requires the vendor to keep detailed records for each person employed including name, address, telephone number, social security number, occupational classification in which worked, hourly wage rate paid, supplements provided, number of hours worked, and actual wages paid. Certified payroll record/s MUST be submitted at the time of invoicing. Effective April 28, 2008, if this is a public work contract covered by Article 8 of the Labor Law, the contractor understands and agrees that the filing of payrolls in a manner consistent with Subdivision 3-a of Section 220 of the Labor Law shall be a condition precedent to payment by the State of any State approved sums due and owing for work done upon the project. Without submittal of certified payroll record/s, payment will not be made. Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 30% Women Owned Sub-Contracting Goal: 7% Service-Disabled Veteran-Owned Business Contracting Goal: 6% SCOPE OF WORK includes, but is not necessarily limited to the following- 1st floor bathroom renovation 1. Contractor is responsible for complete demolition and disposal of construction garbage. Rip out floor tile, subflooring, drywall etc. 2- Repair flooring: Contractor shall provide proper support for new subflooring. All deteriorated mod members (studs and joists) shall be replaced in kind' sister joisted as additional structural support. Floor drain is to be relocated closer to the shower and the floor must be pitched properly towards the drain. Contractor is responsible for providing pictures as proof 3. Install % inch plywood. Plywood is to be glued and screwed to floor joists. Paint the plywood with sealer install waterproof liner that is to be continuously running on all walls at least 1 foot with no cuts to the liner, then cement board. 4. Contractor is to install Porcelain floor tile (Unglazed finish) 12" X 12" Daltile Hastings Gray 12 in. x 5. Replace and install new shower footing , Contractor is to connect new plumbing, shower body and shower head. Deltaln2ition Two-in-One 5-Spray 6.8 in. Dual Wall Mount Fixed and Handheld Shower Head in Brushed Nickel. 6. Installation of the new toilet. Contractor must provide new wax ring and toilet flange. 7. Contractor is to install new drywall and cement board around the shower tape and plaster walls. 8. Contractor is to install a new 90 min fire rate door and frame. Path up to hallway drywall and paint. 9. Wall tile is to be installed on all walls from floorto ceiling. Contractor to use Porcelain glazed tile Home Decorators Collection Kolasus White 12 in. x 24 in. Matte Porcelain Wall Tile. 10. Contractor is responsible to Paint and plaster the ceiling. 1 coat of primer and 2 coats of white semigloss ceiling paint. 0. Contractor is responsible to Paint and plaster the ceiling. 1 coat of primer and 2 coats of white semigloss ceiling paint. 11. Provide and install new light fixture. 2. Contractor to provide a new all wood vanity sink, faucet, new plumbing and water hoses, medicine cabinet, New ceiling vent cover. 13. Install New ADA compliant Grab bars around shower area and the toilet. Grab bars are to be secured to the studs or wooden block must be installed for the proper support. 14. Contractor to install a new stone (quartz) door threshold. ll floor and wall tiles are to be grouted; Clear Silicone is to be applied all around shower as a second coating upon grout curing. 16. Contractor must provide 2 cases of wall and floor tile as a spare. Please note, vendors must remain for the mandatory site visit in its entirety in order to ask questions regarding the specifications of this project. Leaving the site visit early will cause you to be ineligible to submit a quote. Any vendor questions not asked during the site visit must be submitted in writing to: Brooklvn.CMM.Bids@opwdd.ny.gov (within two business days of the site visit: July 29, 2022).
