Description

The Housing Authority of Jefferson County will receive sealed bids for Lead Paint Removal funded under Modernization Program No. IL01P059-501-21 at Project IL-59-1/Mt. Vernon (Mays Manor) in Jefferson County, Illinois. Bids shall be submitted in a single lump sum proposal the following work. IL-59-1/Mt. Vernon (Mays Manor) Work consists of removal of lead paint materials including doors, frames, windows, trim, panels, and soffits, and replacement with new material. In accordance with Clause No. 9, Bid Guarantee, of the Instructions to Bidders, all bids must be accompanied by a negotiable bid guarantee in the form of a certified check or bank draft, payable to the Housing Authority of Jefferson County, U.S. Government Bonds at par value, or a satisfactory bid bond executed by the Bidder and acceptable sureties in an amount equal to five percent (5%) of the bid. The surety company must be authorized to do business in the state where the project is located and must be acceptable to the Government. If the bid guarantee is not submitted with the bid, the PHA will reject the bid. Proposals shall be made on unaltered bid forms which are incorporated herein. All blank spaces shall be completed. Proposals shall be signed with name typed below signature. When bidder is a corporation, proposals must be signed with the legal name of the corporation followed by the name of the state of incorporation and the legal signature of an officer authorized to bind the corporation to a contract. The successful bidder will be required to furnish and pay for satisfactory performance and payment bond or bonds acceptable to the Housing Authority all in accordance with Clause No. 10, Assurance of Completion, of the Instructions to Bidders. Attention is called to the provisions for equal employment opportunity and that payment of not less than the minimum salaries and wages as set forth in the Specifications must be paid on this project. Any State rate that exceeds the corresponding Federal rate is inapplicable and shall not be enforced. All bidders shall submit with each bid, the Contractor's Affirmative Action Certification to comply with the provision for equal employment opportunity. Representations, Certifications and Other Statements of Bidders, Form HUD-5369-A shall be submitted with the bid. Bidders shall carefully examine the documents and construction site to obtain firsthand knowledge of the existing condition. Contractors will not be given extra payments for conditions which can be determined by examining the site and documents. The Housing Authority reserves the right to reject any or all bids or to waive any informalities in the bidding. No bid shall be withdrawn for a period of sixty days (60) subsequent to the opening of bids without the consent of the Housing Authority. Partial sets may be purchased at the cost of photocopying and shipping cost upon completion of a request for partial plans/specifications form and submitting to the Architect. Documents may also be downloaded from the Architect's website for bidding purposes only. Proposal forms and contract documents, including plans and specifications, are on file at the office of the Housing Authority and at the office of Eggemeyer Associates Architects, Inc., 3029 South Park Ave./P. O. Box 640, Herrin, Illinois 62948; Phone No.:(618)988-2380. The work shall be commenced at the time stipulated in the Notice to Proceed to the Contractor and shall be fully completed, including punch list items, within forty-five (45) consecutive Calendar days thereafter.
