Description | The A/E team, including architects, engineers, surveyors, and professional consultants shall perform a building assessment to determine compliance with the Joint Commission Life Safety LS 01.01.01 (EP 2), inspect the West Palm Beach VA Medical Center for Life safety deficiencies according to 2015 NFPA 101 Life Safety Code. Provide comprehensive as-built Life Safety Plans per the Joint Commission Standard LS.01.01.01, EP 3 and including additional items listed in the scope of work below. Team shall provide a list of discrepancies based off their findings and provide recommended methods for corrections. Team shall also update the facilities current life/fire safety plans to reflect existing as-built conditions based off the survey. A/E shall provide inspection reports and findings at all fire and smoke doors and frames fulfilling new Joint Commission Requirement on fire door documentation reports. A/E shall provide training of VA staff on how to inspect fire door and frames for NFPA discrepancies for continued compliance. A/E shall assume all areas of the campus are, and must remain fully operational, and that inspections may require coordination with the Owner for after-hours access, accompanied access and protected access (for medically sensitive areas, etc.), and shall include all such work as part of the A/E Scope of Work (SOW). Life Safety inspection shall include the following. Inspecting all fire walls (including doors, door frames, and hardware) smoke walls, locations of all fire extinguishers, smoke compartments, and exit signs. The survey and drawings shall take place at the West Palm Beach VA Medical and include the following buildings: Comprehensive Overall Site to include buildings, roads, walks, canopies, ramps, parking areas, fire hydrants and Fire Department connections. Bldg. 1 Hospital. Bldg. 3 Community Living Center. Bldg. 15 Cancer Center. Bldg. 16 Mental Health Outpatient Clinic. Bldg. 18 PET/CT. Bldg. 19 Hospice. Bldg. 21 MRI Building. Bldg. 22 Domiciliary. A/E shall reference NFPA 101 and other NFPA applicable codes and provide the follow for each deliverable. Life Safety Plan Requirements. A. Occupancy Classifications (including square footage per classification). B. Door and window locations. C. Door swings. D. Door Ratings in Rated Walls. E. Occupancy. F. Occupancy Count. G. Egress / Exit Locations Indicating Required & Provided Width. H. Exit Signage with Directional Arrows. I. Fire Extinguishers (wall mounted, cabinet mounted, floor mounted, etc.). J. Fire Alarm Pull Stations. K. Fire Alarm Smoke Detectors. L. Fire Alarm Visual. M. Door Hold Open devices tie into Fire Alarm. N. Fire Dampers. O. Elevators, Stairs and Ramps. P. Travel Distance to Exit on Common Path of Travel. Q. Exit Location Separation Respective to Overall Diagonal Length of Building. R. Dead End Corridors. S. Locations of all Fire Walls. T. Locations of all Area Separation Walls. U. Locations of all Smoke Barriers and Smoke Compartments. V. Locations of all Combination Fire Wall / Smoke Barriers. W. Locations of all Smoke Walls. X. Locations of Suite boundaries for Suite of rooms with internal corridors not part of exit access corridor system (both sleeping and non-sleeping). Y. Suite Names including square footage of suite. Z. Locations of all Areas of Refuge. AA. FACP Locations. BB. Room Name and Number. CC. Legend that clearly defines all features of fire safety. DD. Locations of all chutes and shafts. EE. Locations of all Hazardous Storage Areas. FF. Identify any approved equivalencies or waivers. Fire Door & Frame Report Requirements. Location of doors and frames. Time of inspection.
