Instructions for Proposal Submittal: 1. Offerors shall provide an electronic copy of their proposal via email by COB December 29, 2021. The proposal shall be submitted in PDF and any part of the proposal illustrating calculations shall be submitted in Microsoft Excel, if available. Responses to the solicitation must include the following Total price of the project Resume of the proposed key personnel Past performance history of similar contracts 2. Evaluation Factors: NON-PRICE EVALUATION FACTORS Non-Price Evaluation Factor: Key Personnel Offerors shall submit a resume of the proposed key personnel to perform under the resulting contract that demonstrate that the proposed personnel meet the minimum qualifications for the position as outlined in the SOW. Ensure certifications are listed and described in detail. PAST PERFOMANCE Non-Price Evaluation Factor 4: List of project s of similar scope completed Offerors shall provide at least three projects completed of services similar in scope to this requirement. the government reserves the right to retrieve recent (within the past 5 years from the date of issuance of this RFP) and relevant Past performance information from any sources available to include, but not limited to, PPIRS or other databases; interviews with Program Managers, Contracting Officers from any Federal Agencies. Past performance ratings will be reviewed in all categories of performance history of contracts within the last 5 years PRICE Price Evaluation Factor Offerors proposal shall include a firm fixed price for 180-day project completion. The firm fixed price shall be inclusive of, but not limited to, all direct labor expenses, materials, training, insurance coverage, travel, subcontracts, other direct costs, overhead, general and administrative expenses, and profit. All items must be addressed fully and meet the requirements as listed in the solicitation and SOW. Offering different terms, other prices or different response times then as listed will be deemed insufficient and unresponsive and will not be considered for award. Magnitude of Construction: Between $25,000 and $100,000 *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document. Location- James J Peters VAMC 130 W Kingsbridge Road Bronx New York Contractor shall furnish all labor, material, and equipment to replace two (2) roof top exhaust fans and stacks and make all electrical and mechanical connections and secure with structural supports. This is a turnkey installation and the contractor is responsible for any additional wiring, conduit, supports, etc that may be required for proper installation.
