Yondota Rd, Curtice, Ohio. The work to be complete includes the removal of the existing 3-in HPDE piping including accessories and existing diffuser rings at the Intake Crib. The contractor will also reconfigure the process piping within the Surge Well and replace the filters and drain piping for the existing strainers in the Permanganate Building. The completed project will include the installation of three 3-in Steel Reinforced HDPE pipelines from the Surge Well to the Intake Crib, including associated mounting brackets and accessories. The approximate distance between the Surge Well and Intake Crib is 15,493 linear feet. The Contractor will be required to provide 48,807 linear feet of steel reinforced HDPE pipe and approximately 295 linear feet of stainless steel braided hose. City forms must be filled out and attached to the bid: Project Paperwork (Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate); Signed Contract by Vendor CEO/President; Current Worker's Compensation Certificate; Insurance Certificate listing the city of Toledo as an additional insurer; and Child Support Affidavit. The following items will be required upon awarding the contract: Any additional Insurance or Bonding as Required. Deposit to guarantee bid.
