Description

Special instructions for Flooring Installation A. All carpet installations and repairs will comply with the Carpet and Rug Institute Carpet Installation 104 and 105 Standards dated September 2015 with September 2021 updates. B. All vinyl composition tile (VCT) and plank flooring installations will comply with the manufacture’s installation standards. C. All rubber base installations will comply with the with the manufacture’s installation standards. D. Contractor is responsible for verifying all field measurements prior to installation. E. Contractor must be able to accommodate all required installations including, but not limited to, stairs, ADA accessibility, commercial, and residential. F. Contractor is responsible for the off-site disposal of all removed carpeting and padding, excess flooring material and construction debris immediately following installation. G. Contractor must complete all installations within three business days of receipt for task orders under $2,000 and within five business days of receipt for task orders exceeding $2,000. H. Contractor is required to offer a full color selection for commercial glue down and carpet square installations. I. DHA reserves the right to issue multiple contracts as a result of this solicitation. J. All pricing contained in Section B Fee Schedule includes materials and labor necessary for installation. K. All carpet installations will meet all city, state and federal guidelines, codes and requirements with regard to environmental, life safety and fire safety regulations. L. DHA reserves the right to award multiple contracts as the result of this solicitation. If multiple contracts are awarded, the DHA Contracting Officer Representative will rotate work to ensure an equitable distribution of work. M. Only the DHA Contracting Officer and Contracting Officer Representative are authorized to order services under this contract. N. The following is the anticipated procedure for issuing work to the selected contractor(s) under this contract. a. Task Orders i. DHA Field Staff at each development will issue open purchase orders to each contracted firm inclusive of all items contained under Section B fee Schedule. ii. DHA field staff will issue individual task orders to one of the contracted firms via e-mail with the installation address, scope of work and estimated quantities. iii. After completion of the work, the contract firm will invoice for services recording the property address and PO number. iv. DHA field staff will verify the work is complete, pricing matches the contract fee schedule and quantities are accurate. If approved, DHA staff will submit the invoice for payment
