Description | DFCM ISF Snow Removal for AB Stores #1 & #2 he following is an INVITATION TO BID for SNOW REMOVAL SERVICE for the; Areas to be Serviced: AB Store #1, located at 205 West 500 South, Salt Lake City, Utah 84101 AB Store #2, located at 1154 Ashton Ave. Salt Lake City, Utah 84106 Contract Period: Contract starting date September 1, 2022 to August 31, 2027. Specifications are included as part of this invitation. Minimum Requirements to bid: 1. Proof of business license required with bid submittal. 2. Supply an Insurance Certificate with the bid submittal. 3. Bidders shall provide a resume of their experience as well as current names, address, e-mail address and telephone numbers of three (3) references to which the bidder has provided services during the past five years. In addition, if bidders have worked at DFCM managed buildings, the respective DFCM Facility Coordinator(s) will be added to the references and will become part of this evaluation. Bids will be evaluated on cost of services, provided that past experience of bidder and bidder's references are acceptable. Contract Requirements: A. Snow shall be removed from driveways and parking areas at the location after an accumulation of 2 inches or as requested and shall be accomplished prior to 7:00 a.m. or per request by DFCM Coordinator. B. Contractor shall provide all equipment and supplies to perform work required in this specification. Manner and Time Conducting Work: A. All work shall be performed by individuals directly employed and supervised by the snow removal contractor hereinafter referred to as Contractor. (NO SUBCONTRACTORS ARE ALLOWED) B. Contractor shall be required to submit copies of all signed time tickets, indicating hours worked to the State. C. All work performed other than that specifically called for herein shall be done only after authorization by DFCM and shall be a rate quoted by Contractor. D. Contractor shall be required to carry adequate insurance coverage for his employees, equipment and liability coverage for any damages that may occur. Method of Payment: Payment shall be made not more often than once each month. Payment shall be made in a timely manner through the Division of Finance. Cancellation: The contract agreement may be terminated under, but not limited to the following conditions. Description A. The contract shall remain in effect on the same terms and conditions. It may be terminated by either party at least sixty (60) calendar days prior to the termination. B. In the event the Contractor, through the result of workmanship or his organization, fails to perform the services agreed to under this contract, the State may serve written notice upon the Contractor of its intentions to terminate said contract.
