Description

The Work is to perform water service line replacements, containing lead-based hazards, at residential units that qualify for the Wayne County Medicaid Lead Hazard Control Community Development (LHCCD) Grant Program. The following overviews contractor responsibilities: 1. Survey property, review reports, and provide quote, based on contracted pricing schedule, for water service line replacements at identified residences before beginning work. 2. Replace water service line, or portions of line as identified, containing lead-based hazards. 3. Work with Wayne County staff to establish and implement a schedule for lead service line replacement services. 4. Establish procedures for program implementation, assisting Wayne County personnel with addressing any issues that may arise on site. 5. Monitor service line replacement activities with the goal of meeting program benchmarks. 6. Prioritize tasks to meet needs within the allotted timeframe. 7. Establish and monitor replacement schedules to meet planned unit completion deadlines. 8. Maintain contact with Wayne County personnel regarding replacement progress. 9. Obtain all applicable work permits and coordinate with local municipalities, regulatory agencies, coalitions, and action agencies for the duration of service line replacement work. 10. Restore disrupted areas to like, or better, condition. 11. Provide contractor and supplier invoices to Wayne County personnel for payments within fourteen days of unit completion. 12. Develop and maintain quality assurance standards for all work done based on local municipality, state, and EPA regulations on service line replacement. The following overviews County responsibilities: 1. Identifies residential homes where work is to be performed. 2. Approves quotes for work to be performed. 3. Program management will provide contractors with overall project guidance. The program administrator will act as a liaison between the Wayne County Health Department and the contractor providing overall administration, review of project documentation, and guidance on meeting program objectives and benchmarks. 4. Periodically reports on project operations to stakeholders. 5. Represent the project to the state, county, local agencies, and the media. 6. Maintains records for all units accepted for service line replacement.
