Description

The City of Mayfield (OWNER) will receive proposals through Codell Construction Company (Construction Manager as Contractor) to furnish all labor and materials to complete the Demolition and Disposal of City of Mayfield Fire Station #1 located at 104 N. Sixth Street, Mayfield, KY 42066 The work for this project is related to Severe Storms, Straight-Line Winds, Flooding and Tornadoes that occurred in Western Kentucky on December 10th and December 11th, 2021 which resulted in Presidential Declaration of Disaster dated December 12, 2021, 4630-DR-KY. This is an acknowledgement that FEMA Financial Assistance Awards will be used to fund a portion of the Project Contract and a portion will be funded by the American Rescue Plan Act. All Bidders shall comply with all applicable federal law, regulations, executive orders, FEMA policies, procedures, and directives including, but not limited to those found in the Volume One Project Manual Bidding and Contract Documents. Proposals shall be presented through competitive sealed bidding and opened publicly pursuant to KRS 45A.080 and 2 C.F.R.Part 200. Contract shall be awarded to the responsive and responsible bidder. Contract shall be executed between the bidder and Codell Construction Company, the Construction Manager As Contractor. Each bid must be accompanied by a 5% Bid Bond. The bid will be submitted in duplicate on copies of the Form of Proposal located in the Volume One Project Manual Bidding and Contract Documents. Both copies of the Form of Proposal and Bid Bond shall be placed in a manila envelope and have the Project Name and Proposed Bid Package number affixed. No Bidder may withdraw a bid submitted for a period of sixty (60) days after the date set for the opening of bids. All questions related to the bidding documents should be directed to the Construction Manager As Constructor. The Project Schedule will be issued via addendum. The contract will include liquidated damages assessed at $1,000.00/day if the completion date is not met. Successful Bidders are required to furnish a 100% Performance and Labor and Material Payment Bond for this Project.
