1. Hot and cold mix price per ton F.O.B. your plant 2. Mowing of Putnam, Highland, Rainbow cemeteries a. Price per mow your equipment per cemetery on an as per needed basis. b. Should include: i. Mowing and trimming. ii. Reseeding and fertilizing new grave sites or any area that is in need of maintenance. iii. Leveling gravesites as needed. iv. Emptying of trash barrels and removal of dead flowers from gravesites. v. Resetting of fallen grave markers. vi. Proof of Worker`s Compensation coverage. vii. Certificate of Liability Insurance 3. Mowing and trimming of the Oak Grove Recreation Area (this includes park and grass areas surrounding the school) a. Should include: i. Mowing and trimming. ii. Proof of Worker`s Compensation coverage. iii. Certificate of Liability Insurance 4. Limestone prices per ton F.O.B. your plant. 5. Dust control and sealer price per gallon F.O.B. your plant. 6. Gravel price per ton F.O.B. your plant. 7. Asphalt price per ton applied as directed by the Trustees. 8. Chip & Seal, price per square yard for single, double and fog seal as applied according to the township specifications.
