Description

The MICC End User requires the following items, Brand Name or Equal, to the following: LI 001: Audiometric Single Wall Booth, One-Person Booth. Test positions divider curtains made of antibacterial material. One testing positions with heavy duty padded stool. Booth Exterior Color Linear White, Khaki industrial nylon carpeting, Khaki exterior color, Khaki industrial nylon carpeting. Double glazed, acoustical, safety glass with observation window in door size: 1’-8” wide X 4’-6” high with 1/4” thick laminated safety glass for both inner and outer door. Inner and outer door will swing out with hinges on the right. Door 36” width x 73 ¾” Height Units are not grounded.Two 120v recessed LED light fixtures. Window between Control and Exam room. Jack Panel with Phone Jacks and USB ports. 2 Set of HVAC Collars for suites. Double wall design, double magnetic seal with heavy duty hinges.Ventilation systems in audiometric test booths must provide adequate air exchange for patient comfort and must not interfere with the required allowable noise levels when turned on. Low-profile, sound and vibration isolated floor system. Installation of booth must be completed by manufacture trained and certified installer. Certification of booth is required upon completion of installation.Inside-8’8” x 6’1” Outside 10’8”x 8’2” - pre-drilled interior and exterior hole for installing fire suppression capabilities. Must be compatible with BENSON CCA200 mini audiometers. Type 1 sound level meter requirements of ANSI Standard S1.4 and S1.4A Amendment (or latest approved ANSI standard). Advanced Features: Capable of standard diagnostic/comprehensive hearing evaluation battery and auditory processing battery, DOEHRS-HC,SPRINT, etc., as well as the HINT, which is completed through soundfield testing (i.e., specialized testing). Handicap accessible. Window on side of booth for viewing the patient during testing., 1, EA; LI 002: Installation Turnkey installation Post-award site evaluation required by certified installer with SRRC staff, Medical Maintenance, Safety and Facility Engineer. Must provide architectural drawings of audio booth and site requirements. Vendor must provide all necessary labor, material, parts, equipment for installation, off-loading and inside delivery to required site/locations. Vendor must be able to install equipment outside of normal business hours, during the weekend, or be flexible to adjusting as needed by the customer. Vendor must provide all labor and tools required for installation of contracted equipment. Vendor is required to define the need for staging area to accommodate delivery of booths. Responsible for removal and disposal of excess trash and materials.
