Description

Generally, the work includes installation of new sidewalks, fence, aluminum bleachers, and asso-ciated site work. Additionally, the work includes a brick entrance and gate at each field. Bids shall include: 1. Properly completed Proposal Form in duplicate. All Contractors planning to bid shall submit to the Architect Contractor's Qualification State-ment (AIA Form A305), Certificate of Insurance with contractor's current limits, proposed con-struction superintendent's resume, and list of completed projects of similar scope within the past 5 years including project cost, description of work, timeline, and Owner and Architect ref-erences prior to 5 days bidding. Bid Deposit in the amount of 10% of Bid. 3. Not less than the prevailing hourly wage rate as determined by the Illinois Department of La-bor pursuant to Illinois Revised Statutes, Ch. 489, Sec. 39s-1 et. seq. All Bidders are advised that all work of this Con-tract shall be subject to the provisions of the "Il-linois Prevailing Wage Act". Prevailing wage rates have been determined and are on file at the Office of the State of Illinois Department of Labor, tele-phone (217) 782-6206. All Bidders shall submit and enclose with their bid a notarized certification that the Contractor is not barred from bidding on the Contract as a result of a violation of either Section 33 E-3 or 33 E-4 of Article 33E, Public Act 85-1295 (Senate Bill 2002) effective January 1, 1989. Certification is located in the Pro-posal Form in the Project Manual. Bids shall be accompanied by a bid bond in the amount of 10% of the Base Bid, made payable to the Jersey C.U.S.D. #100. Bids not accompanied by a bid bond will be rejected. Successful Bidder will be required to provide Sure-ty Performance Bond and Payment Bond, each equal to 100% of the Contract cost on prescribed forms within ten (10) days after notice of accep-tance is issued by Owner. Surety on bond shall be by a duly authorized U. S. Treasury Backed company licensed to do business in the State of Illinois, acceptable to Owner. The Board of Education for Jersey C.U.S.D. #100 reserves the right to reject any or all bids and re-serves the right to waive any irregularity in bidding, which in their considered opinion is warranted. No bid shall be withdrawn for sixty (60) days after opening of bids
