This project will also include roadway markings. Bid Bond 5.00% Payment Bond 0.00% Performance Bond 100.00% Project Duration 80 days All bids must have the required paperwork and bid guaranty attached with this e-bid in order to be deemed responsive. If you are e-bidding and are using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The bid guaranty may be a bid bond or a certified check or cashier's check on a solvent bank. Forms with Bid Submission City forms must be filled out and attached to the bid: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; Signed Contract by Vendor CEO/President; Certificates of Insurance; Current Workers Compensation Certificate; Court-Ordered Child Support Affidavit; Independent Contractor PERS forms; and additional Insurance as Required. ODOT requires a DBE Utilization plan to be submitted prior to bid opening. The following items will be required prior to awarding the contract: The apparent low bidder must submit C-92s and a DBE Affirmation form to ODOT as well. Prevailing Wages are required for this project. It is the obligation of a contractor or subcontractor to make payment in accordance with the prevailing wage determinations. Only pre-qualified contractors are eligible to submit bids for this PROJECT. Pre-qualification status must be in force at the time of bid, at the time of award, and through the life of the construction contract. For work types that ODOT does not pre-qualify, the LPA must still select a qualified contractor. Subcontractors are not subject to the pre-qualification requirement. The "prime" contractor must perform no less than 30 percent of the total original contract price. |
