Each bid shall be accompanied by a completed, notarized application form and submitted on the enclosed proposal form. Each bidder, by making a bid to furnish the aforementioned item(s), signifies their intention and good faith to enter into contract with the City of Waukegan should they be awarded the bid. Each proposal must be accompanied by a letter from such a surety company stating that it will execute Bonds, on forms provided by Owner, upon award of the Contract to Bidder.
