Description

The bid documents shall also include a non-collusion affidavit, a certification of non- segregated facilities, and an equal employment opportunity form, all properly executed, signed, sealed and notarized (if indicated). Each bid shall be accompanied by a bid bond, certified check, cashier's check, bank treasurer's check or bank money order in the amount of five per cent (5%) of the total bid submitted. Each bidder shall provide evidence of a Drug Testing, Policy as stated in the General Instructions to Bidders. No bid shall be with drawn after the opening of the bids without the expressed con sent of the Vanderburgh County Commission for a period of thirty (30) days after the scheduled time of the bid opening. The Contractor receiving the award will be required to furnish a performance bond, Indiana Form 86A revised in 1947, in the amount of one- hundred percent (100%) of the award amount, which is to remain in effect for twelve (12) months from the date of acceptance of the completed work. The Contractor will also be required to furnish a payment bond on this project. - The Contractor shall list any subcontractors and the percentage of the contract to be performed by each subcontractor in the bid submittal. - Construction and in stallation shall be in full accordance with the contract drawings, specifications, special provisions, and applicable sections of the current edition of the Indian Department of Transportation . Standard Specifications and any current Supplemental Specifications to the current edition Standard Specifications. The Vanderburgh County Commissioners reserve the right to reject any and/or all Bids and waive any informality in the bidding. Scope of Work The intent of this project is to provide concrete patches on all roads in Westwood Hills subdivision at the intersection of Upper Mt Vernon Road and Boehne Camp Road, Hunter's Ridge Subdivision on Browning Road south of Boonville New Harmony Road, in Whispering Hills subdivision off of Hwy. 65 and off of St. Wendel Road, and on Pleasant View Drive between Briar Court and Greendale Drive. The Contractor is encouraged to contact the County Engineer for any clarification or questions. Line item descriptions shall be considered as estimates, and work added or deleted will be reviewed after the contract is awarded. Any increases in quantities will be paid based on the unit prices provided by the Contractor on the Itemized Proposal. The Contractor will be paid only for the work performed and material used as approved by the County Engineer. No work will be approved for payment unless the County Engineer has been notified of the proposed work. The Contractor shall be responsible for notifying the County Engineer at least one day prior to all proposed work days. The completion date for all work is May 12, 2023. The award of the contract will be based on the low bid provided for the total cost of all roads included in the contract
