Description

The contractor shall furnish all labor, equipment, materials, and supervision to complete the replacement of door and card readers at Salt Lake City VA Medical Center, located at 500 Foothill Drive, Salt Lake City, UT,84148. Project magnitude: between $25,000 and $100,000. The NAICS code is 236220 - Small Business size standard is $39.5 M. Important Notice: Apparent successful offerors must apply for and receive verification from the Department of Veterans Affairs Center for Verification and Evaluation (CVE) in accordance with 48 CFR Part 74 and VAAR 819.70 by submission of documentation of Veteran status, ownership, and control enough to establish appropriate status. Offerors must be both VISIBLE and VERIFIED by the Department of Veteran Affairs CVE at the time of offer submission. Failure to be both VERIFIED by CVE and VISIBLE on VetBiz at the time of offer submission and contract award will result in the offeror's proposal being deemed non-compliant. All offerors are urged to contact the CVE and submit the required documents to Obtain CVE verification of their SDVOSB. status if they have not already done so. In accordance with FAR 52.204-7, System for Award Management (SAM), Prospective contractors shall be registered in the SAM database prior to close of solicitation. All submitted proposals must be received in email by the closing date and time. This solicitation Closes August 19, 2022 @ 10:00 A.M. (MT). Proposals shall be submitted via e-mail i accordance with Part I / Section B, to be considered for this project. Proposals received in any other format will result in the offeror's proposal not receiving further consideration. Project Title: Building 42 & 3 Door Replacement Location: Building 42 & 3 exterior doors, Salt Lake City, UT, George E. Wahlen, Depart. of Veteran Affairs Medical Center I. SCOPE OF WORK: A. Furnish all necessary services, equipment, labor, materials, supervision, and tools to remove the existing metal doors and frames and replace them with FRP ballistic rated doors and associated jambes. The hardware will need to be replaced with like items closures, locksets, strikes, etc. B. The contractor will need to install 2 card readers for the doors and 3 sensors tied into the system to alert the boiler plant of door opening. 2 single entry doors and 1 garage rollup door. Conduit will need to be run for the control wires from Building 1 data closet next to room 1C14-1. There will be some excavation needed when crossing from B1 to 42. Utility's will need to be located and marked prior to trenching. Safety will be notified and adhered to during this project. C. The Prime Contractor shall designate a minimum of one Site Safety and Health Officer (SSHO) to administer the contractor's safety program and government-accepted Accident Prevention Plan. Each subcontractor shall designate a minimum of one Competent Person (CP) in compliance with 29 CFR 1926.20 (b)(2) that will be identified as a CP to administer their individual safety programs. Also, The SSHO or an equally qualified designated representative/alternative will maintain a presence on the site during construction operations in accordance with FAR Clause 52.236-6: Superintendence by the Contractor. CPs will maintain presence during their construction activities in accordance with above mentioned clause. A listing of the designated SSHO and all known CPs shall be submitted prior to the start of work as part of the APP with the training documentation and/or AHA. In order to comply with above, please do NOT bid for same SSHO that would or would potentially cover multiple projects on the campus. I.2A SELECTION PROCEDURES This acquisition will be a competitive acquisition utilizing FAR Part 15 source selection employing tradeoff source selection procedures. Competing offerors' past performance history will be evaluated on a basis of significantly more important than, cost or price considerations. By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations, and certifications. All such offers shall be treated equally except for their prices and performance records. Failure to meet a requirement may result in an offer being determined non-compliant. It is the intent of the Government to award without discussions; however, the Government will reserve the right to have discussions if such are needed.
