Description

1 Roof Replacement: Tear off ALL layers of existing shingles (Note: There are multiple layers, including cedar shingles) and replace the entire roof. Replace any underlying rotten or damaged roof deck boards, and install 7/16" OSB sheathing over the entire deck. Install 'W' flashing at all valleys. Provide ice and water shield a minimum of 3 feet wide at all roof edges, and valleys, install synthetic roofing felts, and metal drip and apron flashing. Install new roof vents per code. Provide all necessary flashings, and minimum 30 year dimensional shingles 2 Downspout Modification: At the South side of the house, move the downspout located near the middle of the house, to the West end of the house. (Patch the opening at the former location.) Install a new downspout which chases down the wall, and then is directed out over the sidewalk, and support it with a wood structure. 3 Chimney Repairs: Remove the chimney to below the roof deck, and deck-over. C V 4 Front Entry Porch (wood): Remove and replace the ENTIRE porch structure. Install new 6x6 wood posts and footing. Install new stairs, and a front entry deck with guardrails, per code. Install an upper deck with rubber roof and guardrails. Install perimeter trim boards, and skirting. Install a finished ceiling above the lower deck. Wrap the posts, and beams in finished trim work. Install finished flashing as necessary between the new construction, and the existing walls/siding to create a water-tight assembly. All construction is to be code compliant. (PERMIT), CV, Pb-N 5 Window and Door Trim Wrap: Wrap all exposed door and window trim in pre-finished sheet metal. Apply color matched oil based sealant at all seams, edges, and over nail heads. (Note: The first floor windows are being replaced via another program. Coordinate the timing of this repair with the rehab specialist.) CV, Pb-N 6 Jump Porch Painting: Scrape, prime paint, and finish paint the entire rear jump porch. Colors per owner. CV, Pb-N Miscellaneous: The contractor is responsible to field verify all measurements, the amount of materials needed, and the number of windows in the building. If any item in this scope calls for a certain manufacturer, model number, or approved equivalent of a particular item, and that item is to be substituted, both NIDC and the owner must approve the substitution in writing as a part of the contract. Both the "Rehabilitation and Technical Specifications and Performance Standard for the City of Milwaukee - February, 2006" and the "NIDC Addendum to Specifications, " dated 8/23/07 & 5/13/08, are incorporated into this scope by reference. These items provide an outline of materials requirements & performance expectations. Updated copies can be obtained from NIDC @ 809 N Broadway-3rd Flr. Lead Related Work: Any task that a disturbs previously painted surface shall be performed by properly certified personnel and follow the State of Wisconsin Department of Health Services Administrative Code Chapter DHS 163 regarding the certification for the identification, removal and reduction of lead-based paint hazards (Pb). And, if applicable: Any task that disturbs a previously painted surface requires Milwaukee Health Department notification (Pb-N). Window replacement shall be performed by properly certified personnel and requires a Milwaukee Health Department Permit (Pb-A). YES NO 7 Jump-Porch, Door Replacement: Replace the existing main door at the rear jumpporch with a metal clad exterior door with a high upper-lite. Install a thumb-turn deadbolt and locking handle. Paint all exposed wood trim work; jambs and brick molding, etc. CV, Pb-N 8 Vinyl Replacement Windows - Lead Abatement (First Floor): Replace existing woodsash first-floor windows with matching vinyl replacement windows. Install color matched sealant at all window edges. CV, Pb-A 9 Basement Window Covers: Replace all rotten OSB panels at the basement window opening. Finish paint the new panels covers. C V INTERIOR GENERAL 10 Lead (Pb) Clearance: The contractor is to contact and coordinate with the Milwaukee Health Department to obtain lead testing clearances. The contractor is to conduct all necessary cleaning at the property in order to obtain passing lead swipe tests. Pb-N 11 Plaster and Paint Repairs: Remove loose paint at all areas which possibly contribute to lead (Pb) contamination. Patch all areas of damaged (gouged, water damaged, etc.) existing painted wall/ceiling finishes, and damaged woodwork. All patches are to match the existing finish of the adjacent surfaces. Spot prime paint and spot finish paint the patched areas with closely matching paint. These repairs must be performed as required to receive MHD Lead Clearances. CV, Pb-N 12 Alarms: A.) Smoke Alarms; Install a smoke alarm in the basement, first floor, second floor, per code. (One in each "area", one in each bedroom, one outside the bedroom areas, and one in the common rear stairwell. Estimated unit quantity is 12. All smoke alarms shall be sealed battery, 10 year lithium ion units. B.) Carbon Monoxide Detectors; Install a CO detector in the basement, and one in each first/second floor kitchen area. Quantity of CO detectors is three (3). CV, Pb-N 13 Stair Handrails: Install finished (painted) stair handrails at all flights of stairs from the basement to the second floor. CV, Pb-N BASEMENT 14 Electrical Cover Plates: Install outlet, switch, and junction box cover plates at all locations where missing. C V 15 Abandoned Electrical: Remove or properly terminate all conduit and BX cable, which have electrical wires within. C V 16 Water Heater, Note: The existing water heaters are being replaced through a different program. By Others 17 Furnace Ductwork: The existing furnaces do not have ducted returns. Install hard ducted metal returns for both furnaces, per code. Reuse existing ductwork and grilles where possible and new components, where necessary. C V 18 Furnace Repair: The existing North furnace has water incorrectly draining into the interior mechanicals and wiring. Correct and eliminate the source of the water, and replace any damaged parts, as necessary. C V 19 Water Heater Vent Repairs: Seal the vents of the existing water heaters to the masonry chimney with high temperature sealant. Ensure the vents are connected correctly to the water heaters. C V 20 Gas Valve Cap: Install a cap at the unprotected gas valve, on the South wall. C V
