Prevailing Wages are required for this project. It is the obligation of a contractor or subcontractor to make payment in accordance with the prevailing wage determinations. City forms must be filled out and attached to the bid: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Suspension and Debarment; Primary and Subcontractor Forms; Best Bid Criteria; Signed Contract by Vendor CEO/President ; MBE Participation form; Certificates of Insurance; Current Worker's Compensation Certificate; Court-Ordered Child Support Affidavit; Independent Contractor PERS forms; and additional Insurance as Required.
