Description

The Jicarilla Health Clinic in Dulce, NM has requirement for safe removal and disposal of bio-hazard waste services in complex industry of its own and must comply with multiple mandatory government regulations. WORK REQUIRED: 1. The contractor shall provide transportation and medical waste disposal of infectious waste including sharps containers in accordance with all applicable Federal, State, Local laws and regulations including protection of government personnel, property and general public. 2. The contractor shall provide red hazardous waste containers with lids and having a minimum of 44 gallon capacity and bags large enough to handle all medical waste, kept in the container. The containers must be approved and conform to OSHA blood borne pathogen standards and all Federal, State and local standards for the handling and disposition of hazardous medical waste. 3. The contractor shall clean and disinfect empty containers which shall be free of any residual. 4. The contractor shall schedule monthly pickup of hazardous waste on a date agreed upon with the Government Contracting Officer's Representative. Additional pickups shall be provided which may require firm fixed rate. 5. The medical waste containers shall be kept at a designated location identified by the facility. 6. Certification/Licensure: The contractor shall be certified and state licensed authorized to handle, transport and properly dispose of medical infectious waste in accordance with the standards and requirements of the Colorado Department of Environmental Quality and all applicable health department(s). Evidence of this certification and/or licensure shall be provided to the Contracting Officer and maintained in an active and current status throughout the performance period. The contractor shall inform the COR of any changes which may arise in applicable Federal, State, County and tribal regulations or licensure requirements that may affect infectious medical waste. 7. The contractor shall prepare and provide copies of all required documents necessary to the Contracting Officer prior to the commencement of services to ensure compliance with all applicable Federal, State and other regulatory standards and regulations. 8. The contractor shall properly label and handle containers used for collecting and transportation of all medical waste in accordance with Federal, State and local regulations and standards. 9. Manifest shall be provided for the pickups and disposals including the number of pounds processed for biomedical waste including all sharps containers. 10. The contractor is responsible for the preparation of all documents including medical waste tracking forms, manifest and shipping reports which all shall include the number of pounds of waste removed, number of containers removed and number of containers replaced. Documentation of services shall include all authorized signatures on referenced documentation and carried with all shipments to the authorized disposal location(s). 11. The contractor is required to provide legible signed and dated documentation including Medical Waste Tracking Forms, Manifest Shipping Reports attesting to servicing of medical waste, as non-hazardous. Documentation shall include the number of pounds including the disposal site name, date and representatives signature, if different from the contractor. Waste streams and corresponding documentation shall be identified and tracked for each facility site and shall not be combined with other sites. All signed copies shall be provided to the OCR with 30 days of service rendered.
