Buffalo Psychiatric Center (BPC), Building 4, is located at 400 Forest Avenue, Buffalo, New York 14213 (Site). Scope of work detailed in this document relates to the cleaning and sanitizing of Heating Ventilation Air Conditioning (HVAC) systems and air ducts. Additionally, the scope of work also details the procedures to be used for indoor air sanitizing and removal select sheet rock surfaces Based on Indoor Air Quality (IAQ) investigations and sampling completed on December 5, and December 13, 2019, it was confirmed that the Building 4 west side first and second floors were impacted with high indoor mold spore counts within the breathing zone, inside the air ducts and in the Air Handling Units (AHUs). Based on detected mold spore types and spore counts within the first and second floors located on the west side of the building, New York State Office of Mental Health (NYSOMH) and BPC (site) decided that all of AHUs and air ducts located in the Building 4 west and east sides serving the respective first, second and third floors have been impacted and will need cleaning and sanitizing.
