Description

The BASE BID work includes: The work includes: Construction of a Sanitary Sewer Lift Station and 241 LF of 2" Sanitary Force Main along with public 267 LF of gravity sanitary sewer located in the Riverview Dr. R/W. Work includes sanitary sewer service connections to the R/W line for 4 individual lots, stone access drive to Lift Station, and gate at the end of Riverview Dr. Bids will be based on sum total of all bid items using lump sum and unit bid prices. Contractors must bid all items. Addenda to the bid documents will be posted to Becker Impressions. Bid security in the form of a certified check or a cashier's check in an amount equal to 5% of Bidder's total bid or in the form of a Bid Bond in an amount equal to 10% of the Bidder's total bid and payable to City of Maumee is required of all Bidders. Performance Bond and Payment Bond, equal to the amount of the Contract, will be required of the bidder awarded the Contract. A two-year Main- tenance Bond will be required from a period beginning upon completion of the Contract work. Bidders must comply with the regu- latory and licensing requirements of the State of Ohio. No bid may be withdrawn for a peri- od of 60 days after the opening of bids. The Owner reserves the right to reject any or all bids, select any or all projects, and to waive any informality or irregularity in any bid. The Owner reserves the right to add or subtract from the quantities shown in the bid/contract documents. Domestic steel use requirements as specified in Ohio Revised Code 153.011 apply to this project. Copies of 153.011 can be obtained from any of the offices of the department of admin- istrative services or through electronically The prime bidder and significant subcontractors shall be ODOT prequali- fied in their respective work areas. The Contractor must secure and maintain valid Ohio workers' compen- sation coverage until the project has been finally accepted by the owner. A certificate of coverage evidencing valid workers' compensation coverage must be submitted to the owner before the contract is executed. The Contractor must immediately notify the owner, in writing, if it or any subcontractor fails or refuses to renew their workers' compensation coverage. Furthermore, the Contractor must notify the owner, in writing, if it or any of its subcontractor's workers' compensation policies are canceled, terminated or lapse. The failure to maintain valid workers' compensation coverage shall be consid- ered a breach of contract which may result in the Contractor or subcontractor being removed from the project, with- holding of pay estimates and/or termi- nation of the contract. In accordance with Ohio Revised Code $153.03 and during the life of this project, the Contractor and all its Subcon- tractors that provide labor on the Project site must be enrolled in and remain in good standing in the Ohio Bureau of Worker's Compensation ("OBWC") Drug- Free Workplace Program ("DFWP") or a comparable program approved by the OBWC. Prevailing Wage Rates are applica- ble to this Project.
