Description | Preventative Maintenance on HVAC for LPD Preventative Maintenance on Rooftop Air Cooled Chillers, Air Handlers, Boiler, Liebert Units, and Air Conditioner Units Lafayette Police Department, 900 East University Avenue, Lafayette, LA 70503; Lafayette Police Department Precinct 4, 1820 Moss Street, Lafayette, LA 70501; Lafayette Police Department Defensive Tactics Building, 1818 Moss Street, Lafayette, LA 70501; Lafayette Police Department Firearms Range, 350 Michael Ann Drive, Lafayette, LA 70501 Preventative Maintenance on multiple units at the locations listed above as described in the Items to Bid below. Contractor must be able to service as well as maintain. Work must be performed between the hours of 8:00 a.m. until 5:00 p.m., Monday thru Friday, except for holidays observed. This maintenance work shall require the contractor to make quarterly inspections, with additional work entitled ANNUAL INSPECTION, to be performed during the fourth quarter inspection or as indicated in the specifications. All items must be bid on in order to be considered a responsive bid. This work shall be awarded to one vendor based on total extended price for items 1-12. Any questions regarding the specifications must be addressed to Thomas Metrejean at tgmetrejean@lafayettela.gov. Only questions in writing shall be addressed. Please include in the email's subject line - Questions - LPD HVAC. Please note attached insurance requirements sheet. The successful contractor will be required to furnish attached certificate of insurance, including Item I - Workman's Compensation with statutory limits and Employer's Liability with minimum coverage of $500,000; Item II - Commercial General Liability with minimum coverage of $500,000;and Item III - Business Auto Liability with minimum coverage of $500,000. Items V and VII - LCG listed as Additional Insured on both Liability Policies and Waiver of Subrogation for the Workman's Compensation - shall be worded EXACTLY as they appear on the attached certificate of insurance all for approval of Lafayette Consolidated Government. It is suggested that you speak with your insurance agent, prior to submitting a quote, concerning the insurance and endorsements required by these specifications as there could be additional premiums associated with the requirements. Effective January 1, 2011, all insurance companies must use an "ACORD form" unless they have obtained permission from the commissioner of insurance's office. Only "ACORD or approved Certificates of Insurance" will be acceptable. The insurance form bound in specifications is for information purposes only. Quotes will be evaluated by the Purchaser based on the lowest responsible quote submitted which is also in compliance with the specifications. The Lafayette Consolidated Government reserves the right to reject any and all quotes or any portions thereof, to waive informalities and to select the material that best suits its needs. The Lafayette Consolidated Government strongly encourages the participation of DBEs (Disadvantaged Business Enterprises) in all contracts or procurements let by the Lafayette Consolidated Government for goods and services and labor and material. To that end, all contractors and suppliers are encouraged to utilize DBEs in the purchase or sub-contracting of materials, supplies, services and labor and material in which disadvantaged businesses are available. Assistance in identifying said businesses may be obtained by calling Shaun Williams (337-291-8410). Any bidder not furnishing bid security when required, may cause rejection of said bid. The successful bidder's bid security Shall Equal Five Percent (5%) of the Bid Price or the Amount Specificed in the Notice to Bidders, and will be retained until a satisfactory contract has been entered into between the bidder and the Lafayette City-Parish Consolidated Government. |
