Description | Contractor is responsible to provide all labor, transportation, materials, apparatus, tools, equipment, services, and supervision necessary for the Asbestos Abatement Project located at the VA Boston Healthcare System,West Roxbury VAMC located at 1400 VFW Parkway, West Roxbury, MA 02132. There is asbestos-containing floor tile and mastic in rooms A-138 and A-144 in Bldg 2. This floor tile and mastic will need to be abated from these two areas. The contractor is to fully abate and demo the following designated areas of asbestos: a. Building 2, Room A-138 (work space): Estimated 703 square feet of floor tile and mastic. b. Building 2, Room A-144 (work space): Estimated 625 square feet of floor tile and mastic. The contractor shall: 1. Meet with VA Facilities staff and Asbestos Coordinator, review existing drawings and estimates, and perform field work necessary to determine required materials and equipment. The VA would like to schedule this meeting during the week of March 21, 2022. 2. Permits and licenses are the responsibility of the contractor. This includes the 10- day Notification to MADEP. Contractor will provide a copy of the 10-day Notification to the Asbestos Coordinator. The Notification form will contain the correct name/address of the facility and identify the specific Building/Room Number or location of the abatement. 3. Asbestos contractor must be licensed by the Commonwealth of Massachusetts Department of Labor Standards as an Asbestos Contractor. All licenses must be sent to the Asbestos Coordinator prior to commencing work. 4. All asbestos containing materials will be removed from designated areas. 5. The abatement area will be vacant and all furniture and equipment removed prior to contractor arriving on site by VA personnel. 6. Work will be in accordance with applicable VHA, OSHA, EPA, and Mass DEP standards and regulations. Requirements for OSHA to be completed by contractor. 7. Contractor will provide an independent CIH to monitor and conduct environmental testing during and after asbestos abatement. 8. Follow all the requirements for dust control as listed in the Infection Control Risk Assessment. Work under negative pressure and following the OSHA requirements for asbestos as applicable. 9. Provide asbestos abatement certification for all areas abated. 10. Includes disposal. Asbestos waste will be placed in appropriate containers and shipping by a licensed hazardous material / waste approved transporter (Department of Transportation). Ensure the waste is profiled to be accepted at designated (by contractor) facility. Manifests will be signed by an approved VA employee. Contractor to provide an original copy and end disposal facility return copy to the Asbestos Coordinator. |
