Description

For the furnishing of the necessary materials and construction of the LUC-Monroe St & Silica Dr Upgrd (Part I), PID 107489 LUC-Silica Drive Bridge Replacement (Part II), PID 111563 The proposed work consists of: PID 107489 - Upgrade and widening of the intersection of Monroe Street and Silica Drive including new asphalt pavement, concrete walk, curbing, drainage, earthwork, pavement markings and traffic signal replacement. Project also includes upgrading the at-grade crossing with Norfolk Southern Railroad. PID 111563 - Removing the existing bridge and pedestrian bridge on Silica Drive over Tenmile Creek and replacing with a three-span prestressed composite concrete box beam superstructure with reinforced concrete deck on cap and column piers on semi-integral concrete abutments, and necessary approach work. All bidders must be pre-qualified with the Ohio Department of Transportation for the type of work required by this project. During the bidding for the project, Contractors MUST submit their DBE information electronically to the Ohio Department of Transportation (ODOT)'s Local Let DBE website at time of bid. Modification of DBE information will not be allowed after bid opening. Failure to submit the DBE information thru the website will constitute an incomplete bid All bids shall be signed and submitted on the blanks which are bound in the bidding documents. Bid blanks must be returned in the bound documents. Bids shall state the unit prices on the blanks provided therefore; shall be enclosed in a sealed envelope marked - "BID FOR LUC-Monroe St & Silica Dr Upgrd (Part I) and LUC-Silica Drive Bridge Replacement (Part II)". Bids received after the scheduled bid opening date and time will neither be read nor considered and will be returned unopened to the bidder. The City of Sylvania reserves the right to reject any and all bids and to waive any irregularity in any bid, to determine the lowest and best bidder, and to waive any informalities in bidding. The bid guaranty may be of two forms: 1. A Bid Guaranty and Contract Bond using the form in this booklet. (The amount of the bid does NOT have to appear on this form.) 2. A certified check, cashier's check or letter of credit in favor of the City of Sylvania in the amount of 10% of the bid. If the contract is awarded, a Contract Bond will be required, which is a 100% payment and performance bond. Upon acceptance of the work, a one year maintenance bond in the amount of 25% of the contract will be required to guarantee such work. No bidder may withdraw his bid for a period of 60 days after the scheduled closing time for the receipt of bids.
