Version: 10/18/2022 Page | 1 of 6 Request for Proposals Grounds Maintenance Services I. Introduction. Davis County Government ("the County") owns and operates 17 different facilities and campuses throughout Davis County The County intends to continue providing pleasing outdoor spaces for patrons, the public, and employees. The County also intends to continue supporting water efficiencies and savings through proper maintenance, which extends the life expectancy of the landscaping at all of its facilities with the assistance of a Grounds Maintenance Contractor. Each facility and campus is unique in design, plantings, layout, and grounds maintenance needs. It is the intent of the County to contract with a licensed, competent, and professional Grounds Maintenance Service Provider (the "Service Provider") for 5 years of services. II. Scope of Work. Grounds Maintenance Contractor Services : Provide and furnish all contractor duties, work, labor, cost, supplies, materials, equipment, tools, labor, sub-contractors, means & methods, fees, vehicles, rentals, shop cost, taxes, insurance, salaries, and all other expenses related to the Grounds Maintenance of the 17 facilities The County is in need of landscape maintenance services including: A. Mowing with bagging; B. Xeriscaping/planter beds; C. Hardscape cleanup (garbage and light debris); D. Fertilization & weed control; E. Aeration; F. Spring cleanup; G. Fall cleanup; H. Basic shrub trim Grounds Maintenance service, as described and shown in Exhibit D, for the following 17 locations: A. Davis County Administration, 61 South Main Street, Farmington; B. Farmington Library, 133 South Main Street, Farmington; C. Health Department, 22 South State Street, Clearfield; D. Memorial Courthouse, 28 East State Street, Farmington; E. North Davis Senior Center, 42 South State Street, Clearfield; F. Central Davis Senior Center, 81 East Center Street, Kaysville; G. Children's Justice Center, 125 South Main Street, Farmington; H. Animal Care & Control, 1422 East 600 North, Fruit Heights; I. DMV 969 West Clark Lane, Farmington; J. Davis County I/M Tech, 520 West Old Mill Lane, Kaysville; K. B. Complete and submit the Applicant Proposal Form, attached as Exhibit B to this RFP, which must be signed by an authorized representative of the company. C. Company Background/Experience. Please demonstrate how the Applicant's history, organization, and partnerships distinguish it from other Applicants. Careful attention should be paid to providing information relevant to Davis County's needs, identified in this RFP. Include concise responses to the following: 1. Describe the Applicant's history, including the number of years in business under the current organizational name, structure, and services offered. 2. Provide details of the company organization structure, manpower, key personnel, tools, equipment and machinery that will be used on this Project. 3. Provide information on the Applicant's other annual service contracts in the past three years. 4. Are the Applicant's audited or otherwise verifiable financial statements available upon request 5. Is the Applicant involved in any pending litigation that may affect its ability to provide its proposed solution or ongoing maintenance or support of its products and services If so, please explain. D. References. Provide at least three, but no more than five, customer references for which the Applicant has recently provided or is currently providing services that are similar in size and scope to those sought through this RFP. Please provide the following for each identified reference: 1. Customer name, address, and telephone number; 2. Name, title, address, email, and telephone number of the Applicant's primary contact with the customer; 3. Describe the services or products provided to the customer; and 4. Service start and end date. Each of the references provided by the Applicant will be contacted by the County and sent a standard electronic questionnaire form to complete and submit to the selection committee. The County reserves the right to modify the following projected schedule for this RFP process at its sole discretion Last day to submit Questions Feb, 1, 2023 5:00 pm County may reject any proposal submitted in response to this RFP if one or more employees, agents, representatives or otherwise contacts any other County employee, agent, or representative regarding this RFP. |
