Tuba City Outpatient Treatment Center Fire Alarm Systems Inspection, Repair, Replace or Install Project

The Division of Behavioral and Mental Health Services (DBMHS) is requesting evaluation/installation and inspection services for a non-existing fire alarm control panel system at the Tuba City Outpatient Treatment Centers in Tuba City, Arizona. It is the intent of DBMHS to bring all fire alarm systems up to date and in compliance with building and life safety codes. The vendor must be licensed as a commercial fire safety inspection company. Locations: Tuba City Outpatient Treatment Center, Tuba City, AZ Building Property #8352 approximately 7,000 sq. ft. This project entails the following: 1. The installation of an addressable fire alarm control panel and components (including but not limited to relay modules; pull stations; smoke detectors; interior horn alarm; interior horn/strobe). Tie into an existing hydraulic fire sprinkler system and exterior bell alarm. 2. Provide as-built drawings for the fire protection plan. 3. Connect to a fire alarm monitoring system. 4. Inspect and certify fire risers and sprinkler suppression systems and its components. 5. Provide a pipe obstruction inspection, a flow test and inspect the backflow preventer. 6. Recommendation of special fire suppression systems in the data room for sensitive computer and other electronic equipment. 7. Inspect and certify four (4) fire extinguishers to include recharge/ rebuild if needed per NFPA 10. 8. Inspect and certify seven (7) exit lighting and emergency exit lighting and signage. 9. Inspection of four (4) existing exit doors with panic release hardware (in good working order). 10. Location, ownership and testing of nearest public fire hydrant. 11. Provide project manager with detailed Inspection reports and language that can be utilized for the development of a scope of work for each site for repairs, replacement or additions. This report is to include costs. 12. Upon completion of inspection services, minor repairs equal to or less than One Thousand Dollars ($1,000.00) allowance per year (make sure this is included to the cost proposal). 13. Annual inspections of all mentioned systems for five (5) years to ensure proper operations of all systems and components. 14. Provide all materials (operating/maintenance manuals) and workmanship warranty information to site. 15. Provide operating and maintenance information to project manager and site staff 16. Contractor is responsible for field verification of systems at this site. 17. Cost details breakdowns are to include labor, equipment, materials, minor repairs allowance, travel expenses and the Tuba City Sales Tax of 6%. Please be cognizant that the grand total is the contract total. 18. Exclusions: Based on your expertise- please list any and all fire related life safety code requirements not otherwise in this scope of work. (Do not list costs with main proposal).
