Description

\St. Mary's County Public Schools (SMCPS) is requesting price quotes for the material, service, tools, delivery, and labor required to furnish and install the replacement of the well pump at Hollywood Elementary School. The condition of the well pumps reliability has deuterated and SMCPS plans to replace this pump before a pump failure. The current well was installed under permit number SM-95-1722, please find attached the Pump "As Built", Pump Set Test report, Pump Curve, Well Survey Report "2006", original well permit, etc. o This work will include the isolation and removal of the existing well pump and associated piping, installation of a new Goulds 35GS50 submersible pump with a M50434 motor designed to replace the existing well pumps performance. o Installation will include (2) two - (4") four inch stainless steel check valves, 3/4 " poly water level tube, and 1/4 " stainless steel airline and other materials and services germane to the installation. o Pump test will be completed to verify performance by the contractor. o The contractor will provide labor and material to sanitize the disrupted portions of the potable water system. o SMCPS will engage an independent firm to take and analyze water samples after sanitization. o The contractor will be responsible for all permits as needed. WARRANTY - The product specified shall be free from defects in materials and workmanship for a period of 1 year and for a period of 2 years for all parts. Warranty period shall start from date of installation. o Vendor will provide a signed W9 with quote. o Vendor will complete the attached Business Relationship Affidavit and submit with their response o Work will be scheduled with SMCPS, Department of Maintenance, and performed between the hours of 6:30 a.m. until 3:00 p.m., Monday through Fridays except when "all schools and offices closed" as indicated on the SMCPS operational calendar. The SMCPS' calendar and school closing updates are available for view at website No Vendor will be allowed to offer more than one price for each item or service even though the Vendor may feel that they have two or more choices that meet specifications. Vendors must determine for themselves which to offer. If the Vendor should submit more than one price on any item or service, all prices for that item or service will be rejected. o Vendor shall ensure that all employees report to the main office in school buildings daily to sign-in and sign-out while work is being performed o General terms and conditions are attached.
