Senior Center LVT Flooring

The White County Board of Commissioners is requesting sealed proposals from qualified contractors to provide LVT flooring and installation services at the White County Senior Center All questions concerning this RFP MUST BE SUBMITTED IN WRITING, (email is preferred but fax and mail may also be used) to the Finance Assistant no later than 12:00PM, May 9, 2023. Misti Byrd, Finance Assistant White County Board of Commissioners 1235 Helen Highway Cleveland, GA 30528 mbyrd@whitecounty.net Fax: 706-865-1324 SCOPE OF WORK The White County Board of Commissioners is requesting sealed proposals from qualified contractors to provide LVT flooring and installation services at the White County Senior Center, 1239 Helen Highway, Cleveland, GA 30528. The contractor shall furnish all labor, equipment and materials, and perform all operations necessary to remove and install flooring materials. The contractor will be responsible for measurements and is responsible for accuracy of the measurements and the fit of the work. Contractor shall provide layout plans, work plans and schedules, to be approved by White County staff. The contractor shall provide original packaging listing the manufacturer's name, product name, identification numbers and any other related information. The contractor shall provide written instructions and on-site training on the maintenance of the flooring for the custodial staff. Contractor shall: a. Take up existing carpet from offices and hallways where needed b. Take up existing VCT from rooms and hallways where needed b. Take up existing VCT from rooms and hallways where needed c. Perform floor prep and perform patchwork if needed d. Install 20mil LVT - color will be determined by White County staff e. Install 4.5" Vinyl Cove Base - color will be determined by White County staff f. Install transitions as needed The county will provide a dumpster for disposal of flooring materials.
