Description | **As of June 30, 2022, Project has been awarded. Award amount is $85,242.56** All bids shall be presented to the City Clerk before this time. The proposed work shall be completed within 45 calendar days plus State holidays from the Notice to Proceed date, and includes the following major items: Milling of 1,560 SY of Existing Road Surface Installation of 1,560 SY of HMA 9.5M64-10R Surface Course, 2" Thick Removal and Replacement of 440 Feet of Concrete Curb Plans. No refund shall be made for the return of such documents. Direct any questions to the City Engineer at Bryson & Yates, LLC at (856) 589-1400. Non-Collusion Affidavit; and a Consent of Surety as to a Maintenance Bond as per N.J.S.A. 40A:11-16.3b. All blank spaces in the Proposal, except as otherwise noted, must be filled in; and no change shall be made in the phraseology of the Proposals or of the item mentioned. All information that may be requested of the bidder in these specifications shall be included with this proposal. All erasures, interpolations, or other physical changes on the bid form shall be initialed or signed by the bidder. All applicable surety bonds or LOC required must be written by a surety company listed on the Federal Treasury List (Department Circular 570-Surety Companies Acceptable on Federal Bonds) and such surety company must be acceptable to the City Solicitor and authorized to do business in the State of New Jersey. Pursuant to N.J.S.A. 10:5-33; "Bidders are required to comply with the requirements of P.L. 1975, c.127"; N.J.A.C. 17:27 (Affirmative Action), P.L. 1963 C150 (New Jersey Prevailing Wage Act); and Americans with Disabilities Act of 1990 (42 U.S.C. 12101, et seq). The Contractor is notified that he must comply with P.L. 1977, c. 33 and submit a Disclosure Statement listing Stockholders with his bid. The Contractor is further notified that he must comply with P.L. 1999 c. 238 Public Works Contractor Registration Act and he and any subcontractors must be registered in accordance with this act. The Contractor must also comply with P.L. 2004 c. 57 and submit proof of their New Jersey Business Registration and a copy of the Certificate for any named subcontractors in accordance with the act. This contract is subject to the provisions of the New Jersey Public Contracts Law, N.J.S.A. 40A:11-1, et seq. Bids must be prepared on the standard proposal forms provided, in the manner designated therein. Each bid must be enclosed in a sealed envelope, bearing the name and address of the bidder, and labeled "2019 CDBG - Resurfacing & Safety Improvements Project South Maple Avenue" and shall be addressed to the "Clerk, City of Woodbury". The Owner reserves the right to reject any and or all bids, to waive any informality , and to accept the bid, which in its judgment, best serves the interest of the Owner.
