Description | Bid Proposal Forms: All bidders must use the enclosed Bid Form and Agreements (or copies thereof) to submit their Bid Proposal. No alternate or substitute pricing forms will be accepted. All Proposal responses must be typed or written in ink. Submittal of Bids: All Bid Proposals must be submitted in sealed envelopes that are clearly marked with the following information: "Bid# 2022-10-17 Street Resurfacing". Proposals must be delivered, no later than the Submittal Deadline as specified in the Notice to Contractors, to the following address: City Clerk's Office Ozark Municipal Complex 275 North Union Avenue Ozark, Alabama 36360 Any questions regarding this project may be directed to Ryan Campbell, Public Works Director, City of Ozark, office telephone (334) 774-7032. Inquiries by mail may be sent to: Post Office Box 1987, Ozark, Alabama 36361 or by e-mail at rcampbell@ozarkal.gov Before submitting bids, each bidder must examine the bid documents, including without limitation, all the Contract Documents, thoroughly; familiarize him/herself fully with all State and Local Laws, ordinances, rules, codes, and regulations affecting the work; and correlate his/her observations with requirements of the bid document. The total pricing for the project will be the main determining factor for bid award. The City of Ozark reserves the right to accept or reject all bids or any portion thereof. Work shall be performed between the hours of 7:00 a.m. and 7:00 p.m. on Monday-Friday. All weekend or holiday work and all other deviations from this Work Schedule must be approved by the Public Works Director prior to performance of such work. The entire project shall be completed in sixty (60) calendar days. We request that the project begin on or about 7, November 2022. Time is the essence of the Contract. Any delay in the completion of the work required by the Contract Documents may cause inconvenience to the public and loss and damage to the Owner including but not limited to interest and additional administrative, inspection and supervision charges. By executing the Contract, the Contractor agrees that the Contract Time is sufficient for the achievement of Substantial Completion. Failure to complete the work to the satisfaction of the City of Ozark or to complete the work within the agreed time schedule may result in liquidated damages being assessed at $250.00 per day as needed. The Contractor warrants to the Owner that all materials and equipment furnished under the Contract will be of good quality and new, except such materials as may be expressly provided or allowed in the Contract Documents to be otherwise, and that none of the work will be defective work. The Contractor agrees to warranty all work for a period of one year at the completion of the Contract. A Bid must be accompanied by Bid security made payable to the Owner in an amount of .5. percent of Bidder's maximum Bid price. The security shall be in the form of a certified check, bank money order, or a Bid bond issued by a surety company. The successful bidder and any subcontractor of the bidder shall require their insurance carrier, with respect to all insurance policies, to waive all right of subrogation against the City. The successful bidder shall submit certificates or other documentation to the City with the signed agreement, attesting to insurance coverage for Worker's Compensation Insurance $1,000,000 and General Aggregate $2,000,000, Automobile Liability $2,000,000. The Certificate of Insurance to be provided by successful bidder and any subcontractors to the City with signed agreement.
